Dengue Fever (Dengue) – Mosquito-borne viral infection

Dengue fever is an infectious disease caused by dengue viruses, primarily found in tropical and subtropical regions. It is mainly transmitted by infected Aedes mosquitoes, particularly Aedes aegypti and Aedes albopictus (Asian tiger mosquito).

The disease can be asymptomatic or relatively mild, but it can also lead to a severe course of dengue with bleeding, circulatory failure, and organ involvement.

Pathogen and Transmission

Dengue is caused by four closely related dengue virus serotypes:

  • DENV-1

  • DENV-2

  • DENV-3

  • DENV-4

The viruses belong to the Flaviviridae family.

Transmission primarily occurs through the bite of an infected Aedes mosquito. Direct human-to-human transmission does not occur in daily life.

Incubation Period

After the bite of an infected mosquito, symptoms typically appear after a few days.

The disease often begins suddenly with high fever and a pronounced feeling of illness.

Typical Symptoms

Possible symptoms include:

  • sudden high fever

  • severe headache

  • pain behind the eyes

  • pronounced muscle and limb pain

  • joint pain

  • fatigue and weakness

  • nausea and vomiting

  • skin rash

  • loss of appetite

Due to the sometimes very strong muscle, bone, and joint pain, dengue was formerly also known as "breakbone fever".

Changes in Blood Count

In dengue, characteristic changes in blood values can occur, particularly:

  • decrease in platelets (thrombocytopenia)

  • decrease in white blood cells (leukopenia)

  • in severe cases, changes in hematocrit due to fluid shifts

However, a low platelet count alone does not determine the severity. The overall clinical situation is crucial.

Critical Phase

Particular attention is required when the fever subsides after a few days.

A drop in fever in dengue does not always mean that the disease is over. For some patients, the critical phase begins precisely at this point.

Due to increased vascular permeability, fluid can leak from blood vessels into the surrounding tissue.

Warning Signs for Severe Dengue

Important warning signs include:

  • severe or persistent abdominal pain

  • repeated or persistent vomiting

  • bleeding from the nose or gums

  • vomiting blood

  • blood in stool or black stool

  • increasing weakness

  • pronounced restlessness or unusual drowsiness

  • shortness of breath

  • circulatory problems

  • fluid accumulation

  • signs of organ involvement

These symptoms require immediate medical assessment.

Severe Dengue

A severe course can be accompanied by significant fluid loss from blood vessels, severe bleeding, or organ involvement.

Possible complications include:

  • dengue shock

  • severe bleeding

  • liver involvement

  • heart involvement

  • neurological complications

  • circulatory failure

A severe course of dengue can be life-threatening and requires inpatient or intensive care treatment.

Secondary Dengue Infection

After an infection, longer-term immunity develops primarily against the respective dengue serotype.

A subsequent infection with a different serotype is possible. Certain secondary infections can be associated with an increased risk of a severe course.

Diagnosis

Diagnosis is based on:

  • symptoms and clinical examination

  • stay in a dengue endemic area

  • blood count

  • specific laboratory diagnostics

Depending on the time of the illness, direct virus detection or antigen tests, or later antibody tests, can be used, among others.

Other tropical infectious diseases can cause similar symptoms and may need to be ruled out.

Treatment

There is no specific antiviral standard therapy available for dengue. Treatment is mainly supportive.

Particularly important are:

  • adequate and controlled fluid intake

  • monitoring of circulation

  • monitoring of blood count in cases of appropriate disease severity

  • treatment of fever and pain

  • close monitoring for warning signs

In severe dengue, inpatient treatment with intravenous fluid therapy and intensive monitoring may be necessary.

Caution with Pain Relievers

If dengue is suspected, acetylsalicylic acid (ASA/aspirin) and non-steroidal anti-inflammatory drugs such as ibuprofen or diclofenac should be avoided without medical advice, as they can increase the risk of bleeding.

Paracetamol is often used for fever and pain treatment, provided there are no individual contraindications and the permissible dosage is observed.

Protection against further mosquito bites

Infected individuals should protect themselves as consistently as possible from mosquito bites even during the illness.

A mosquito can pick up the virus from an infected person and subsequently infect other people.

Suitable protective measures include:

  • effective mosquito repellents

  • long clothing

  • mosquito nets

  • window screens

  • reduction of open water accumulations as breeding sites

Dengue after Travel

Fever after a stay in a tropical or subtropical area should be medically investigated.

In addition to dengue, other diseases such as malaria should also be considered, depending on the travel destination. Malaria can also be life-threatening and must be quickly ruled out if there is a relevant travel history.

Homeopathy according to Dr. Banerji

In homeopathy according to Dr. Banerji, specific homeopathic medicines and potencies are used according to the Banerji protocols for various febrile diseases and the associated symptoms.

A reliable treatment of dengue virus infection, prevention of a severe course of dengue, or treatment of dengue shock with homeopathic remedies is not scientifically proven.

Particularly the critical phase of a dengue illness can develop rapidly. Therefore, necessary medical monitoring, fluid therapy, or inpatient treatment must not be replaced or delayed by homeopathic remedies.

Important Note

If dengue is suspected, especially with severe abdominal pain, repeated vomiting, bleeding, shortness of breath, circulatory problems, pronounced weakness, confusion, or a significant deterioration after the fever drops, seek immediate medical attention.

In case of fever after a tropical trip, a possible malaria or other treatable infectious disease should also be medically ruled out.
