Acne (Acne vulgaris) – inflammatory disease of the sebaceous glands and hair follicles
Acne is a common inflammatory skin condition primarily affecting the sebaceous glands and hair follicles. It is characterized by blackheads (comedones), papules, pustules, and in more severe forms, deeper inflammatory nodules.
Acne most commonly appears on the face, back, shoulders, and chest area, as these areas have a high concentration of sebaceous glands.
Acne is common during puberty but can also emerge in adulthood or persist for many years.
How does acne develop?
Several factors contribute to its development:
-
increased sebum production
-
keratinization disorder at the opening of the hair follicles
-
clogged pores
-
proliferation or altered activity of Cutibacterium acnes
-
inflammatory reactions of the skin
-
hormonal influences
-
genetic predisposition
Particularly, androgens can influence sebum production. This is why acne can become more pronounced during puberty or due to hormonal changes, among other factors.
What skin changes are typical?
Depending on the severity, the following can occur:
-
open comedones (blackheads)
-
closed comedones (whiteheads)
-
red papules
-
pustules containing pus
-
painful nodules
-
deep-seated inflammatory changes
-
pigmentation changes after healed inflammations
-
acne scars
Different forms of acne
Depending on the predominant appearance, distinctions include:
-
Acne comedonica – primarily blackheads and whiteheads
-
Acne papulopustulosa – papules and pustules
-
Acne conglobata – severe form with deep nodules, abscesses, and an increased risk of scarring
Additionally, acne-like skin changes can be caused by medications, cosmetics, or other diseases.
Acne in adults
Acne can persist after puberty or appear for the first time in adulthood.
Especially in women, hormonal factors can play a role. If severe acne develops suddenly along with other signs of androgen excess, a hormonal evaluation may be advisable.
What can worsen acne?
Possible influencing factors include:
-
unsuitable or highly greasy cosmetic products
-
mechanical irritation and pressure
-
squeezing or manipulating skin lesions
-
certain medications
-
hormonal changes
-
stress as a possible exacerbating factor
-
certain dietary factors in some affected individuals
However, acne does not simply result from a lack of skin cleansing. Overly aggressive washing and strong degreasing products can further irritate the skin.
Treatment
Treatment depends on the form and severity of acne.
Treatments include:
-
Benzoyl peroxide
-
topical retinoids
-
azelaic acid
-
antibiotics for certain inflammatory forms
-
hormonal treatment for suitable female patients
-
isotretinoin for severe or treatment-resistant acne
For moderate to severe acne, early dermatological treatment is important to reduce the risk of permanent acne scars.
Skin care for acne
Generally suitable are:
-
mild cleansing
-
non-comedogenic skincare
-
non-comedogenic sun protection products
-
avoidance of aggressive mechanical cleansing
-
avoid squeezing inflamed pimples if possible
Acne scars and pigmentation changes
After more severe inflammatory acne, permanent scars can develop. Red or brownish spots can also remain visible for a longer period after the inflammation has healed.
The best prevention against acne scars is early and sufficiently effective treatment of active acne.

In homeopathy according to Dr. Banerji, specific homeopathic remedies and potencies are used for various skin complaints and forms of acne, following the Banerji protocols.
A specific therapeutic efficacy of homeopathic remedies for acne is not sufficiently scientifically proven. In cases of pronounced inflammatory acne or incipient scarring, necessary dermatological treatment should not be replaced or delayed.
Important note
For severely inflammatory, painful, or nodular acne, abscesses, rapid worsening, or incipient scarring, a dermatological evaluation is recommended.
