Dacryocystitis – Inflammation of the Lacrimal Sac and Blocked Tear Duct

Dacryocystitis is an inflammation or infection of the lacrimal sac (saccus lacrimalis). It often occurs when the natural drainage of tears through the nasolacrimal duct is narrowed or blocked. The accumulation of tears allows pathogens to multiply more easily and cause an infection.

This condition falls under disorders of the lacrimal drainage system and the anterior segment of the eye.

How does dacryocystitis develop?

Tears are normally drained through small openings at the inner edge of the eyelid into the tear ducts and then through the nasolacrimal duct into the nose.

If this drainage is obstructed, tear accumulation occurs. Bacteria can multiply in the stagnant secretions and cause acute or chronic inflammation of the lacrimal sac.

Possible causes include:

• congenital narrowing or blockage of the tear ducts

• age-related narrowing

• inflammation and infections

• injuries or scar formation

• changes in the nose and paranasal sinuses

• rarely, tumors or other space-occupying lesions

What symptoms can occur?

Typical symptoms include:

• increased tearing

• painful swelling at the inner corner of the eye

• redness and warmth

• tenderness to pressure

• purulent or mucous discharge

• crusted eyelids

• feeling of pressure in the lacrimal sac area

• in severe infections, fever and general malaise

In chronic forms, pain and redness may be less pronounced, while tearing and recurrent discharge are the primary concerns.

Infections of the anterior segment of the eye

Redness, discharge, and eye pain can also be caused by other conditions of the anterior segment of the eye, such as inflammation of the conjunctiva, eyelids, or cornea. Therefore, a precise ophthalmological examination is important.

Diagnosis and Treatment

Treatment depends on the cause and severity. In acute bacterial dacryocystitis, antibiotics may be necessary. If a tear duct is permanently blocked, surgical restoration of tear drainage may be required.

Severe pain, pronounced swelling and redness of the eye, fever, rapid spread of inflammation, or vision impairment must be immediately evaluated by a doctor or ophthalmologist.
