Buerger's Disease – Thromboangiitis Obliterans / Inflammatory Vascular Occlusions

Buerger's disease, medically known as thromboangiitis obliterans, is a rare inflammatory disease of small and medium-sized blood vessels not caused by classic arteriosclerosis. It primarily affects the arteries and veins of the hands, feet, arms, and legs.

Due to inflammation and the formation of blood clots, the affected vessels can progressively narrow or completely close off. This impairs the blood supply to the tissue.

How does Buerger's disease develop?

The exact etiology of the disease is not fully understood. However, there is a particularly strong correlation with tobacco consumption. The disease occurs almost exclusively in people who currently consume or have consumed tobacco.

It is presumed that in predisposed individuals, an inflammatory reaction of the blood vessels occurs, causing vascular occlusions and circulatory disorders.

What symptoms can occur?

The symptoms arise from reduced blood flow to the extremities. Possible symptoms include:

• Pain in the hands, feet, arms, or legs

• Pain when walking or during physical exertion

• Pain in fingers or toes, even at rest

• Cold hands and feet

• Pale, bluish, or reddened fingers and toes

• Numbness or tingling

• Recurrent inflammation of superficial veins

• Poorly healing wounds on fingers or toes

In severe circulatory disorders, tissue can die (necrosis or gangrene).

Diagnosis and Treatment

For diagnosis, other causes of vascular occlusions, particularly arteriosclerosis, embolisms, clotting disorders, and certain autoimmune diseases, must be excluded. Additionally, ultrasound and vascular examinations may be performed.

The most important measure for Buerger's disease is the complete cessation of tobacco and nicotine use. Even small amounts can promote the progression of the disease. Further treatments depend on the extent and symptoms and primarily serve to improve blood circulation, manage pain, and treat existing wounds.

Newly occurring severe pain, discoloration, feeling of cold, or poorly healing wounds on fingers and toes should be promptly evaluated by a doctor.
