Hemorrhoids – enlarged vascular cushions in the rectum

Hemorrhoids are enlarged or prolapsed vascular cushions at the junction of the rectum and anal canal. These vascular cushions are a normal part of the body and, together with the sphincter muscle, aid in the fine closure of the anus.

Only when they enlarge and cause symptoms is it referred to as a hemorrhoidal disease.

What symptoms can occur?

Typical symptoms include:

• bright red blood on toilet paper or stool

• bleeding during bowel movements

• itching and burning in the anus

• wetness

• pressure or foreign body sensation

• sensation of incomplete bowel emptying

• protrusion or prolapse of hemorrhoids

• pain, especially with additional irritation or complications

Severe pain is rather atypical for uncomplicated internal hemorrhoids and may indicate another condition in the anal area.

What factors promote hemorrhoids?

Possible contributing factors include:

• chronic constipation

• straining excessively during bowel movements

• sitting on the toilet for extended periods

• recurrent diarrhea

• low-fiber diet

• pregnancy

• obesity

• increasing age

Classification into four grades of severity

Internal hemorrhoids are often classified into four grades:

• Grade I: enlarged, but not visible from the outside

• Grade II: protrude during straining and retract spontaneously

• Grade III: protrude and must be pushed back manually

• Grade IV: permanently prolapsed and cannot be pushed back

Treatment depends, among other things, on the severity and symptoms.

Blood in stool not automatically hemorrhoids

Bright red blood during bowel movements is a common symptom of hemorrhoidal disease. Nevertheless, bleeding from the rectum should not automatically be attributed to hemorrhoids.

Other possible causes include:

• anal fissures

• intestinal inflammations

• colon polyps

• diverticular disease

• bowel tumors

New or recurrent bleeding, in particular, should be medically or proctologically clarified.

Treatment

An important basis is the regulation of bowel movements:

• high-fiber diet

• adequate fluid intake

• avoidance of excessive straining

• regular exercise

• toilet times as short as possible

Ointments and suppositories can temporarily relieve symptoms such as itching and burning, but do not necessarily permanently eliminate larger hemorrhoids.

For more pronounced symptoms, various proctological procedures are available, for example:

• rubber band ligation

• sclerotherapy

• further interventional procedures

• surgical treatment for advanced findings

A reliable regression of advanced hemorrhoids or treatment of heavier bleeding by homeopathic remedies is not scientifically proven. Therefore, necessary proctological diagnostics and treatment should not be replaced or delayed.

Important note

Heavy or recurrent bleeding, black stool, anemia, unexplained weight loss, or a persistent change in bowel habits should be medically investigated.

Suddenly occurring severe pain with a palpable swelling at the anus should also be examined, as, for example, an anal venous thrombosis may be present.
