Wet Age-related Macular Degeneration (wet AMD) is a disease of the macula, the central part of the retina responsible for sharp vision and the ability to see fine details.

Unlike dry AMD, the wet form can progress relatively quickly and lead to a significant impairment of central vision within a short period.

How does wet macular degeneration develop?

In wet AMD, abnormal new blood vessels form under or within the retina. This process is called neovascularization.

The newly formed vessels are often unstable and permeable. This can cause fluid and blood to leak into or under the retina. Swelling and changes occur in the macula, which can damage the light-sensitive cells.

The messenger substance VEGF (Vascular Endothelial Growth Factor) plays an important role in the formation of these pathological blood vessels.

What symptoms can occur?

Typical symptoms include:

• sudden or progressively blurred vision

• straight lines appear wavy or distorted

• letters appear displaced or partially missing

• difficulty reading

• problems recognizing faces

• reduced contrast perception

• dark or blurry spot in the center of the field of vision

• rapid deterioration of central vision

The peripheral vision often remains intact. However, the central vision, which is important for reading and detail recognition, can be severely impaired.

Diagnosis and Treatment

Diagnosis primarily involves examination of the fundus and optical coherence tomography (OCT). Depending on the findings, further examinations of the retinal vessels may be necessary.

The most important established treatment for wet AMD are anti-VEGF medications, which are injected directly into the vitreous body of the eye. They inhibit the effect of VEGF and can reduce the growth and permeability of abnormal blood vessels.

Early treatment can slow down vision loss in many affected individuals and, in some cases, even achieve an improvement. Repeated treatments and regular check-ups are often required.
