Vertigo – Dizziness and Balance Disorders

Vertigo is the medical term for certain forms of dizziness. Those affected have the feeling that their surroundings are spinning or swaying, or that their own body is moving, even though no such movement is actually taking place.

Dizziness can have various causes. It often results from a disorder of the balance organ in the inner ear, the vestibular nerve, or the processing of balance information in the brain.

How does vertigo arise?

For stable balance, the brain simultaneously processes information from the balance organs of the inner ear, the eyes, and sensory receptors in muscles and joints. If this information does not match or if one of these systems is disturbed, dizziness can occur.

Possible causes include:

• benign paroxysmal positional vertigo

• diseases or inflammation of the balance organ

• disorders of the vestibular nerve

• Menière's disease

• vestibular migraine

• certain neurological diseases

• circulatory disorders of the brain

• side effects of certain medications

What forms of dizziness are there?

Depending on the cause and perception, different forms can be distinguished:

• Rotational vertigo: sensation that the surroundings or one's own body is spinning

• Swaying vertigo: sensation of swaying or an unstable surface

• Positional vertigo: short attacks of dizziness with certain head or body movements

• Central vertigo: cause lies in the brain or brainstem

• Peripheral vestibular vertigo: cause lies in the inner ear or vestibular nerve

What symptoms can occur?

In addition to the dizziness itself, other symptoms may occur:

• nausea and vomiting

• balance disorders

• unsteadiness when walking

• involuntary eye movements (nystagmus)

• tinnitus

• hearing loss or feeling of pressure in the ear

• headaches

• visual disturbances

Diagnosis and Treatment

Treatment depends on the cause of the dizziness. A ENT and neurological examination, hearing and balance tests, and further examinations for certain complaints may be necessary for clarification.

For benign paroxysmal positional vertigo, for example, targeted positioning and liberating maneuvers can be used. For other forms, therapy depends on the underlying disease.

Important note

Sudden, severe dizziness should be medically investigated. If paralysis, speech disorders, double vision, severe gait instability, disturbances of consciousness, or other acute neurological deficits occur simultaneously, immediate medical help is required.
