Spondylitis / Axial Spondyloarthritis – HLA-B27-Associated Spinal Inflammation
Axial spondyloarthritis (axSpA) is a chronic inflammatory rheumatic disease that primarily affects the spine and the sacroiliac joints. The classic advanced form is known as Ankylosing Spondylitis or Bechterew's Disease.
The genetic marker HLA-B27 is detectable in many affected individuals and plays an important role in the development of the disease. However, a positive HLA-B27 finding alone does not automatically mean that spondyloarthritis is present.
What is inflamed?
In this disease, the sacroiliac joints, vertebral joints, and tendon and ligament attachments to the spine can be inflamed. The term "inflammation of the vertebral bodies" therefore only partially describes the disease.
Over time, chronic inflammation can lead to bony changes and, in advanced stages of the disease, to increasing stiffening of the spine.
What are the typical symptoms?
Characteristic symptoms include inflammatory back pain, often beginning in young adulthood.
Typical complaints include:

deep-seated back or lower back pain

pain in the buttocks and sacroiliac joints

pronounced morning stiffness

nighttime back pain

waking up in the second half of the night due to pain

HLA-B27 positive
HLA-B27 is a hereditary surface characteristic of certain immune cells. There is a strong connection to various diseases from the group of spondyloarthritides.
Important: Many HLA-B27 positive people remain healthy throughout their lives. Therefore, the laboratory value must always be evaluated in conjunction with symptoms, clinical examination, and imaging.
Other possible symptoms
Spondyloarthritis can affect more than just the spine. Possible symptoms include:

inflammation of other joints

enthesitis – inflammation of tendon and ligament attachments

Achilles tendon discomfort

heel pain
-
dactylitis ("sausage fingers" or "sausage toes")

eye inflammation, especially acute anterior uveitis

Note on eye inflammation
A sudden onset of painful eye redness with light sensitivity and blurred vision may indicate uveitis. This should be examined promptly by an ophthalmologist.
Diagnosis
The diagnosis is not made solely by HLA-B27. Clarification may include:

rheumatological examination

HLA-B27

CRP and other inflammation markers

X-ray of the sacroiliac joints and spine

MRI of the sacroiliac joints, especially for detecting early inflammatory changes
A normal X-ray does not rule out early axial spondyloarthritis.
Treatment
The goal of treatment is to reduce inflammation and pain, maintain mobility, and prevent structural damage as much as possible.
Important components include:

regular exercise and targeted training

physiotherapy
-
anti-inflammatory drugs (NSAIDs)

in case of insufficient disease control, targeted immunomodulatory therapies, such as certain biologics or JAK inhibitors
-
treatment of concomitant eye, skin, or bowel diseases
Regular exercise and exercises to maintain spinal mobility and posture are particularly important.

In homeopathy according to Dr. Banerji, specific homeopathic remedies and potencies are used for various rheumatic and inflammatory disease and symptom complexes according to the Banerji protocols.
Reliable control of immunologically induced inflammation or prevention of structural changes or stiffening of the spine through homeopathic remedies is not scientifically proven. Therefore, necessary rheumatological treatment should not be replaced or discontinued independently.
Important note
In cases of long-standing inflammatory back pain with pronounced morning stiffness and improvement with movement, especially with HLA-B27 positivity, a rheumatological clarification is advisable.
