Shoulder Capsulitis / Frozen Shoulder – Adhesive Capsulitis

Frozen shoulder, medically known as adhesive capsulitis, is a painful condition of the shoulder joint capsule. It initially involves inflammatory changes, followed by thickening, scarring, and shrinkage of the joint capsule.

Characteristic symptoms include shoulder pain and increasing stiffness of the shoulder joint. Both active and passive mobility of the shoulder are restricted.

What symptoms can occur?

Typical symptoms include:

  • increasing shoulder pain

  • pain particularly at night

  • pain when lying on the affected shoulder

  • increasing restriction of movement

  • difficulty lifting the arm

  • problems getting dressed or undressed

  • difficulty washing or combing hair

  • problems reaching behind the back

  • pronounced shoulder stiffness

Especially characteristic is often the restricted external rotation of the shoulder joint.

Typical course in three phases

Frozen shoulder often develops over months and typically progresses through several phases:

  • Inflammatory and painful phase ("Freezing"): Pain increases and the shoulder becomes increasingly immobile.

  • Stiff phase ("Frozen"): Pain may decrease, but the movement restriction remains significant.

  • Thawing phase ("Thawing"): Mobility gradually improves again.

The entire course can last many months to several years. For many affected individuals, mobility improves significantly, but recovery can be slow.

How does a frozen shoulder develop?

In many cases, no clear cause can be identified. This is then referred to as a primary or idiopathic frozen shoulder.

A secondary frozen shoulder can occur, for example:

  • after shoulder injuries

  • after operations

  • after prolonged immobilization of the arm

  • with diabetes mellitus

  • with thyroid diseases

  • in connection with certain other diseases

Consider diabetes

People with diabetes mellitus have an increased risk of developing frozen shoulder. The condition can also occur more frequently bilaterally in them and sometimes have a more persistent course.

Differentiation from other shoulder conditions

Not every shoulder pain is a frozen shoulder. Similar symptoms can be caused, for example, by:

  • rotator cuff injuries

  • calcific tendinitis

  • impingement syndrome

  • shoulder osteoarthritis

  • bursitis

  • tendinopathies

  • cervical spine complaints

Therefore, an accurate diagnosis is important.

Diagnosis

The diagnosis is usually made based on symptoms and examination of the shoulder. Characteristic is the restriction of both active and passive mobility.

X-rays, ultrasound, or MRI may be used to rule out other causes of shoulder discomfort.

Treatment

Treatment depends on the stage of the disease and the severity of symptoms. Treatments include:

  • adapted physiotherapy and exercise

  • pain-relieving and anti-inflammatory medications

  • corticosteroid injections into the shoulder joint, especially in certain early phases

  • if necessary, joint capsule distension (hydrodilatation)

Overly aggressive stretching exercises, especially in the highly painful initial phase, can exacerbate symptoms. Physiotherapy should therefore be adapted to the respective phase of the disease.

In severe, long-standing cases, surgical procedures to release the joint capsule may be considered in selected cases.
