Osteoarthritis is a chronic joint disease characterized by changes and breakdown of joint cartilage and other joint structures. It is one of the most common musculoskeletal disorders and becomes more prevalent with increasing age.

Osteoarthritis is to be distinguished from rheumatoid arthritis. While rheumatoid arthritis is primarily an autoimmune disease with pronounced inflammation, osteoarthritis is mainly a degenerative joint disease, although inflammatory processes can also occur.

Which joints are frequently affected?

In principle, any joint can be affected. Particularly common are:

  • Knee joints – Gonarthrosis

  • Hip joints – Coxarthrosis

  • Finger and thumb joints

  • Spinal joints

  • Shoulder joints

  • Ankle joints

  • Metatarsophalangeal joints of the big toe

How does osteoarthritis develop?

Osteoarthritis develops through the interaction of various factors. The balance between stress and load-bearing capacity of the joint changes, which can progressively damage cartilage and other joint structures.

Contributing factors include:

  • increasing age

  • overweight, especially for knee and hip osteoarthritis

  • long-term heavy or one-sided joint strain

  • previous joint injuries

  • joint misalignments

  • congenital joint changes

  • certain metabolic diseases

  • genetic predisposition

What symptoms can occur?

Typical symptoms are:

  • joint pain during activity

  • start-up pain after periods of rest

  • morning stiffness, usually of short duration

  • restricted mobility

  • creaking or grinding in the joint

  • increasing joint stiffness

  • swelling during inflammatory activation

  • muscle tension and loss of strength

In advanced osteoarthritis, pain can also occur at rest or at night.

Activated Osteoarthritis

Osteoarthritis can temporarily become more inflammatory. This is then referred to as activated osteoarthritis. The joint may be more painful, swollen, and warm to the touch.

However, a suddenly very red, hot, and painful joint should be medically evaluated, as gout, bacterial joint infections, or other inflammatory joint diseases could be underlying.

Diagnosis

Diagnosis is based on symptoms and a physical examination. Depending on the situation, additional diagnostic tools may be used:

  • X-ray examination

  • ultrasound

  • Magnetic Resonance Imaging (MRI)

  • blood tests for unclear inflammatory symptoms

Treatment

Significantly damaged joint cartilage usually cannot be fully restored. The goal of treatment is to reduce pain, maintain mobility and muscle strength, and preserve joint function for as long as possible.

Key measures include:

  • regular, joint-friendly exercise

  • muscle strengthening and physiotherapy

  • weight reduction for overweight individuals

  • pain and anti-inflammatory medication as needed

  • physical therapy measures

  • orthopedic aids, depending on the joint

  • in advanced cases, surgical procedures including joint replacement

Complete rest is usually not advisable for osteoarthritis. Regular, adapted exercise is an important part of the treatment.
