Irritable Bowel Syndrome (IBS) is a common chronic disorder of the digestive system. It is typically characterized by recurring abdominal pain, bloating, and changes in bowel habits, without another organic disease being identifiable as a sufficient explanation for the symptoms.

Irritable Bowel Syndrome can significantly impair quality of life but, unlike chronic inflammatory bowel diseases such as Crohn's disease or ulcerative colitis, does not lead to permanent inflammatory destruction of the intestinal lining.

Typical Symptoms

Symptoms can vary greatly from person to person. Common symptoms include:

  • recurrent abdominal pain or cramps

  • bloating and increased gas

  • feeling of fullness

  • diarrhea

  • constipation

  • alternating between diarrhea and constipation

  • sudden urge to defecate

  • feeling of incomplete bowel emptying

  • altered stool consistency

  • mucus in the stool

  • symptoms that change in relation to bowel movements

Symptoms can persist for long periods and vary in severity.

What are the types of Irritable Bowel Syndrome?

Depending on the predominant change in stool, IBS is classified into different types:

  • IBS-D – IBS with predominant diarrhea

  • IBS-C – IBS with predominant constipation

  • IBS-M – Mixed type with both diarrhea and constipation

  • unclassifiable types

This distinction is important because treatment is tailored to the predominant symptoms.

How does Irritable Bowel Syndrome develop?

Its development is complex and not fully understood. Several factors are likely involved.

Factors under discussion include:

  • altered bowel motility

  • increased sensitivity of the gut to distension and other stimuli

  • changes in gut-brain communication

  • changes in the gut microbiome

  • previous gastrointestinal infections

Diagnosis is made based on the typical symptom profile. At the same time, it must be checked whether there are indications of other diseases.

Depending on symptoms, age, and individual risk factors, examinations may be useful, for example:

  • inflammation markers

  • celiac disease screening

  • stool tests

  • fecal calprotectin to differentiate from inflammatory bowel diseases

  • screening for certain infections

  • colonoscopy, if necessary

  • further examinations if food intolerances or other diseases are suspected

Extensive diagnostics are not necessary for every patient with typical IBS symptoms.

Differentiation from other diseases

Similar symptoms can occur in, among others:

  • Crohn's disease

  • Ulcerative colitis

  • Celiac disease

  • Lactose intolerance

  • Fructose malabsorption

  • Bowel infections

  • Diverticular disease

  • Thyroid disorders

  • certain pancreatic diseases

  • Colorectal cancer

Therefore, newly appearing or unusual symptoms, in particular, should be medically evaluated.

Treatment

Treatment is individually tailored to the predominant symptoms.

Possible measures include:

  • individually adapted diet

  • adequate fluid intake

  • soluble fiber in suitable patients

  • regular physical activity

  • treatment of diarrhea or constipation

  • antispasmodic medications for appropriate symptoms

  • peppermint oil preparations, if necessary

  • for selected patients, a time-limited Low-FODMAP diet under professional guidance

  • treatment of confirmed food intolerances

Methods to influence the gut-brain axis can also be helpful in certain patients.

Diet for Irritable Bowel Syndrome

There is no single IBS diet suitable for all affected individuals.

A food and symptom diary can help identify individual triggers. Very extensive and permanent dietary restrictions should be avoided without proven necessity, as they can lead to an unbalanced diet.
