Poliomyelitis, also known as polio, is a highly contagious infectious disease caused by polioviruses. Despite the name "child paralysis," adolescents and adults can also become infected.

Most infections occur without typical paralysis symptoms. However, in a small percentage of cases, the virus attacks the central nervous system and can damage motor nerve cells in the spinal cord and brainstem. This can lead to flaccid paralysis, some of which remains permanent.

Pathogen

Polioviruses belong to the enterovirus group.

Historically, three wild poliovirus types are distinguished:

  • Poliovirus type 1

  • Poliovirus type 2

  • Poliovirus type 3

Wild poliovirus types 2 and 3 are considered eradicated. Wild poliovirus type 1 has not yet been completely eliminated worldwide.

In addition to wild polioviruses, circulating vaccine-derived polioviruses can occur in regions with insufficient vaccination coverage.

Transmission

Polioviruses initially multiply in the throat and particularly in the intestines.

Transmission primarily occurs fecal-orally, for example, through:

  • contaminated hands

  • contaminated water

  • contaminated food

  • close contact with infected individuals

Infected people can shed and transmit the virus even if they do not experience clear symptoms themselves.

Asymptomatic infection

A large proportion of poliovirus infections cause no noticeable symptoms at all.

This is epidemiologically significant, as the virus can still be transmitted.

Mild course of illness

Some affected individuals experience non-specific symptoms, such as:

  • fever

  • fatigue

  • headaches

  • sore throat

  • nausea and vomiting

  • abdominal discomfort

  • muscle and limb pain

These symptoms can initially resemble a common viral infection.

Non-paralytic poliomyelitis

If the nervous system is affected, aseptic meningitis can develop.

Possible symptoms include:

  • severe headaches

  • neck stiffness

  • back pain

  • muscle pain

  • light sensitivity

  • fever

Neurological involvement requires prompt medical evaluation.

Paralytic poliomyelitis

In rare cases, motor nerve cells are damaged, leading to acute flaccid paralysis.

Characteristic symptoms may include:

  • sudden onset of muscle weakness

  • flaccid paralysis

  • reduced or absent reflexes

  • often asymmetrical distribution

  • muscle pain

  • preserved sensation despite motor paralysis

The legs can be affected particularly often, but arms and other muscle groups are also possible.

Bulbar poliomyelitis

Involvement of the brainstem is particularly dangerous.

This can affect nerves that control important functions.

Possible consequences include:

  • swallowing disorders

  • speech disorders

  • breathing problems

  • respiratory muscle disorders

  • circulatory problems

Involvement of the respiratory muscles can be life-threatening and may require artificial ventilation.

Historical "Iron Lung"

Before the introduction of modern ventilation methods, patients with severe respiratory paralysis were sometimes treated in a so-called iron lung.

This negative-pressure ventilator became a well-known symbol of the severe poliomyelitis epidemics of the 20th century.

Permanent paralysis

Recovery after paralytic poliomyelitis varies.

Some muscle strength may recover over time. However, severe damage to motor nerve cells can lead to lasting consequences, such as:

  • muscle weakness

  • muscle atrophy

  • paralysis

  • deformities

  • restricted movement

  • chronic pain

  • walking difficulties

Physiotherapy and rehabilitation play an important role in addressing remaining functional impairments.

Post-Polio Syndrome

Decades after having paralytic poliomyelitis, some affected individuals may experience a recurrence or increase in symptoms.

This is known as Post-Polio Syndrome.

Possible symptoms include:

  • new or increasing muscle weakness

  • severe fatigue

  • muscle pain

  • joint pain

  • muscle wasting

  • reduced physical capacity

  • occasional swallowing or breathing problems

This is not a new poliovirus infection, but rather a late consequence of previous damage to the neuromuscular system.

Diagnosis

In case of suspected poliomyelitis, a rapid neurological and infectiological clarification, as well as targeted pathogen detection, is necessary.

Depending on the situation, the following may be examined:

  • stool samples

  • throat swabs

  • blood

  • cerebrospinal fluid (CSF)

  • further neurological examinations

Direct detection and accurate typing of the poliovirus are also of great importance for public health protection.

Treatment

There is no specific antiviral therapy available that reliably eliminates a poliovirus infection.

Treatment depends on the clinical picture and may include:

  • monitoring of breathing

  • intensive care treatment

  • ventilation for respiratory paralysis

  • pain management

  • physiotherapy

  • occupational therapy

  • rehabilitation

  • orthopedic aids

In acute paralysis, stabilizing vital functions is the primary focus.

Polio vaccination

The most important protective measure is polio vaccination.

Through global vaccination programs, poliomyelitis has been largely suppressed in most countries.

However, adequate vaccination protection remains important, as polioviruses are not yet completely eliminated worldwide, and imported cases or outbreaks remain possible.

Different polio vaccines and vaccination strategies are used depending on the country and vaccination program.

Polio is not yet eradicated worldwide

Global vaccination programs aim for the complete eradication of poliomyelitis.

However, as long as polioviruses are still circulating, there is a risk of new infection chains, especially in regions with low vaccination coverage. International travel can also facilitate introduction into previously polio-free regions.

In homeopathy according to Dr. Banerji, specific homeopathic remedies and potencies are used according to the Banerji protocols for various neurological complaints, muscle weakness, and paralysis symptoms.

A reliable elimination of the poliovirus, prevention of paralytic poliomyelitis, or restoration of already destroyed motor nerve cells through homeopathic remedies has not been scientifically proven.
