Serous Otitis – Fluid in the Middle Ear / Middle Ear Effusion

Serous otitis, also known as serous otitis media or middle ear effusion, refers to an accumulation of fluid in the middle ear behind the eardrum, without necessarily involving an acute purulent inflammation.

Fluid accumulation usually occurs when the ventilation of the middle ear through the Eustachian tube is disturbed. Middle ear effusion is particularly common in children but can also affect adults.

How does fluid accumulate in the middle ear?

The Eustachian tube connects the middle ear with the nasopharynx and ensures ventilation and pressure equalization. If it is swollen shut or does not function adequately, negative pressure builds up in the middle ear, and fluid can accumulate.

Common causes and contributing factors include:

• Colds and upper respiratory tract infections

• Enlarged adenoids, especially in children

• Inflammation in the nasopharynx

• Allergies

• Eustachian tube dysfunction

• Previous middle ear infection

• Changes in the nasopharynx

• Pressure fluctuations, for example, during flying or diving

What symptoms can occur?

Middle ear effusion often causes no severe pain. Typical symptoms include:

• Dull or diminished hearing

• Feeling of pressure or fullness in the ear

• Feeling of "water in the ear"

• Clicking or crackling in the ear

• Distorted hearing

• Occasionally mild ear pain

• In children, sometimes delayed speech development with long-term hearing loss

Serous otitis in children

Middle ear effusion is particularly common in children because the Eustachian tube is shorter and anatomically oriented differently.

If the fluid persists for a longer period, hearing can be impaired. Therefore, hearing checks are important in cases of prolonged or bilateral middle ear effusion.

Serous otitis in adults

A newly occurring unilateral middle ear effusion in adults should be examined by an ENT specialist. In particular, the nasopharynx should be examined to rule out mechanical obstruction of the Eustachian tube.

Diagnosis and Treatment

Diagnosis is made by examining the eardrum. In addition, tympanometry and hearing tests can be performed.

Many middle ear effusions resolve spontaneously within a few weeks. If fluid accumulation persists for longer and there is significant hearing loss, further measures may be necessary.

Depending on the cause, this may include treating an underlying nasal or throat condition, and for long-lasting, pronounced findings, a small incision in the eardrum or the insertion of an ear tube may be considered.

Antibiotics are not generally necessary for uncomplicated serous middle ear effusion, as there is usually no acute bacterial infection.
