Osteoporosis is a skeletal disease in which bone mass and bone quality decrease. This makes bones increasingly unstable and prone to fractures even with minor stress or a slight fall.

A precursor or milder form of reduced bone density is called osteopenia.

Which diseases and changes fall into this category?

Important disorders of bone stability and bone metabolism include:

  • Osteoporosis

How does osteoporosis develop?

Bone is continuously built up and broken down throughout life. If bone breakdown exceeds bone formation for a prolonged period, bone strength decreases.

What symptoms can occur?

Osteoporosis often initially causes no symptoms. Pain often only appears when bone fractures have already occurred.

Typical consequences can include:

  • Vertebral fractures

  • Back pain

  • Loss of height

  • Increasing kyphosis (rounding of the back)

  • Femoral neck fracture

  • Wrist fracture

  • Bone fractures after minor falls

Vertebrae can also collapse unnoticed.

Diagnosis

  • Bone density measurement using DXA

  • X-rays in case of suspected vertebral fractures

  • Blood tests to look for secondary causes

  • Depending on the situation: vitamin D, calcium, phosphate, and other bone metabolism parameters

Treatment and prevention

The goal is to maintain bone stability and, in particular, to prevent bone fractures.

Important measures include:

  • Regular physical activity

  • Muscle and strength training

  • Adequate calcium intake

  • Adequate vitamin D supply

  • Fall prevention

  • Smoking cessation

  • Avoiding excessive alcohol consumption

In cases of increased fracture risk, specific osteoporosis medications may be necessary. Depending on the situation, bisphosphonates, denosumab, or bone-building therapies may be considered.
