Colon Polyps – Polyps in the Colon and Rectum

Colon polyps are tissue growths that originate from the lining of the large intestine (colon) or rectum. They can occur individually or multiply and differ in size, shape, and tissue type.

Most bowel polyps are initially benign. However, certain polyps can develop into colorectal cancer over years. Therefore, their early detection and removal are an important part of colorectal cancer screening.

What types of colon polyps are there?

The most important forms include:

  • Adenomas – benign glandular polyps with different risks of malignant transformation

  • tubular adenomas

  • tubulovillous and villous adenomas

  • serrated polyps or lesions

  • hyperplastic polyps

  • inflammatory polyps

  • rarely other benign or malignant changes

Not every polyp has the same cancer risk. Crucial factors include size, number, location, and histological findings.

Adenoma-Carcinoma Sequence

Some colorectal cancers develop over years from initially benign adenomas. Genetic changes gradually accumulate in the cells until a malignant tumor can form.

The removal of corresponding polyps during a colonoscopy can often prevent this development.

What symptoms do bowel polyps cause?

Most colon polyps cause no symptoms and are discovered during a screening colonoscopy.

However, larger or unfavorably located polyps can lead to the following symptoms:

  • blood in the stool

  • mucus deposits

  • changes in bowel habits

  • diarrhea or constipation

  • rarely abdominal pain

  • iron deficiency or anemia due to chronic blood loss

Visible or occult blood in the stool should always be medically clarified.

Risk Factors

The risk of colon polyps increases with, among other things:

  • increasing age

  • family history of colorectal cancer or colon polyps

  • certain hereditary polyp syndromes

  • chronic inflammatory bowel diseases

  • obesity

  • lack of physical activity

  • smoking

  • high alcohol consumption

  • certain dietary habits

Familial and Hereditary Polyposis Syndromes

In cases of an unusually large number of polyps or an accumulation of colorectal cancer in the family, a genetic cause should be considered.

These include, for example:

  • familial adenomatous polyposis (FAP)

  • MUTYH-associated polyposis

  • various serrated polyposis forms

In such cases, more frequent screenings and human genetic counseling may be necessary.

Diagnosis

The most important examination is colonoscopy.

During this procedure:

  • the entire colon can be examined,

  • polyps can be identified,

  • smaller polyps can be immediately removed, and

  • tissue samples can be taken for histological examination.

The subsequent histological examination then shows what type of polyp it is and whether cell changes or already malignant changes are present.

Treatment

The most important treatment consists of the complete endoscopic removal of relevant polyps.

Different procedures are used depending on the size and shape. Very large or complex polyps may require special endoscopic procedures or, in certain cases, surgery.

Follow-up after Polyp Removal

After removal, the timing of a repeat colonoscopy depends on the individual risk.

Particular consideration is given to:

  • number of polyps

  • size

  • histological type

  • presence of dysplasia

  • completeness of removal

  • quality of the previous colonoscopy

  • familial risk

The recommended follow-up intervals should be observed, as new polyps can still develop even after complete removal.
