Parkinson's Disease (Morbus Parkinson) – see also Huntington's Disease
Parkinson's disease, also known as Morbus Parkinson, is a chronically progressive disease of the central nervous system. It belongs to the group of neurodegenerative diseases, in which certain nerve cells in the brain progressively lose their function.
Parkinson's primarily affects the control of movements. However, in addition to motor symptoms, numerous non-motor symptoms can also occur.
How does Morbus Parkinson develop?
In Parkinson's, nerve cells in an area of the midbrain, known as the substantia nigra, progressively degenerate. These nerve cells produce the neurotransmitter dopamine, which plays a crucial role in the coordination and control of movements.
The increasing dopamine deficiency impairs movement control. Why the nerve cells degenerate in most affected individuals is not fully understood. It is believed that aging processes, genetic factors, and environmental influences play a combined role.
What symptoms can occur?
Typical motor symptoms include:

slowed movements (bradykinesia)

muscle stiffness (rigor)

tremor, often at rest (resting tremor)

difficulty initiating movements

shuffling or unsteady gait

balance and posture disturbances

reduced facial expressions

softening of speech

difficulties with fine motor tasks
Additionally, non-motor symptoms may occur, such as:

changes in concentration and cognitive performance

disorders of the autonomic nervous system
Parkinson's and Huntington's
Both Parkinson's disease and Huntington's disease (Huntington's chorea) are progressive neurodegenerative diseases that can affect movement control, among other things.
However, the causes and typical movement disorders differ significantly. In Parkinson's, slowness of movement, muscle stiffness, and often tremor are prominent. In Huntington's, by contrast, involuntary, exaggerated movements (chorea) are characteristic. Huntington's is also a clearly genetically determined hereditary disease.
Diagnosis and Treatment
Diagnosis is primarily based on neurological examination and the typical symptom profile. If necessary, further examinations are carried out to rule out other causes.
A cure is currently not possible. However, various medications are available that can significantly alleviate symptoms by specifically influencing the dopamine deficiency and its effects. Additionally, physiotherapy, occupational therapy, and speech therapy can be used.
For certain patients, specialized treatment procedures such as deep brain stimulation may also be considered.
Important Note
New or worsening movement disorders, tremors, muscle stiffness, or changes in gait should be evaluated neurologically.
