Neurofibromatosis and Schwannomas – Diseases of the Nerves and Nerve Sheaths

Neurofibromatoses are a group of rare, genetically determined diseases in which tumors can develop particularly on nerves and nerve sheaths. These tumors are often benign but can, depending on their location and size, impair nerves or other structures.

Diseases in this group primarily include Neurofibromatosis Type 1 (NF1), NF2-related Schwannomatosis, and other forms of Schwannomatosis.

How do Neurofibromatoses develop?

They are caused by changes in specific genes that are normally involved in controlling cell growth. The genetic change can be inherited from a parent or occur for the first time in an affected individual.

Depending on the form, various nerve tumors and other changes may occur.

Neurofibromatosis Type 1 (NF1)

In NF1, so-called neurofibromas can form along nerves. Frequently, café-au-lait spots on the skin are also typical. The manifestation of the disease varies greatly. Some affected individuals experience only minor discomfort, while others may develop larger or numerous neurofibromas and other health problems.

Schwannomas

A schwannoma is a usually benign tumor that originates from the so-called Schwann cells. These cells form part of the protective sheath of peripheral nerves.

Schwannomas can fundamentally occur on various nerves. Depending on their location, they can cause pain, tingling, numbness, muscle weakness, or other neurological symptoms.

NF2-related Schwannomatosis

This hereditary disease is characterized by the occurrence of schwannomas. Vestibular schwannomas on the auditory and balance nerves are particularly typical. These can lead to hearing loss, ringing in the ears (tinnitus), and balance disorders.

Additionally, other schwannomas and other tumors of the nervous system, such as meningiomas, may occur.

What symptoms can occur?

Depending on the form, number, and location of the changes, possible symptoms include:

• palpable nodules along nerves

• pain and nerve pain

• tingling or numbness

• muscle weakness

• hearing impairment or hearing loss

• tinnitus

• balance disorders

• visual or other neurological disturbances

Diagnosis and Treatment

Diagnosis is made, depending on the disease form, based on clinical features, imaging tests such as MRI, and, if necessary, genetic tests.

Treatment depends on the type, size, location, and growth of the tumors, as well as the existing symptoms. Regular follow-up examinations, surgical interventions, and, for certain forms of the disease, other specialized therapies may be possible.

Neurofibromatoses and schwannomatoses should be managed by specialists and regularly monitored. In case of suspected hereditary forms, human genetic counseling may be advisable.
