Myopia magna with macular degeneration – high myopia with macular changes
Myopia magna, also known as high myopia, is a severe form of nearsightedness. It is often accompanied by a significant elongation of the eyeball.
Due to the increasing stretching, the retina, choroid, and other structures of the eye can become thinner and more susceptible to pathological changes. If this affects the macula, the area of sharpest vision, it is referred to as myopic maculopathy or myopic macular degeneration, among other things.
How does myopic macular degeneration develop?
In high myopia, the eyeball is usually significantly elongated. This stretching places mechanical stress on the posterior structures of the eye in particular.
This can lead to changes such as:

thinning of the retina and choroid

degenerative changes in the macula

cracks in deeper retinal layers, known as lacquer cracks

When pathological new blood vessels form in the macula, it is called myopic choroidal neovascularization (mCNV). This can lead to a rapid deterioration of central vision.
Diagnosis and Treatment
Regular ophthalmological examinations are particularly important in high myopia. Examination of the fundus and optical coherence tomography (OCT) are used, among other things, to assess the macula and retina.
Treatment depends on the existing changes. An existing elongation of the eyeball cannot be reversed. If pathological new blood vessels appear in the macular area, for example, Anti-VEGF injections may be necessary.
