Motor Neuron Diseases – Diseases and Damage to Motor Nerve Cells

Motor neuron diseases are a group of neurological disorders in which the nerve cells responsible for controlling voluntary muscles – the so-called motor neurons – are damaged or progressively degenerate.

Motor neurons are located in the brain, brainstem, and spinal cord. They transmit movement impulses from the nervous system to the muscles. If these nerve cells are damaged, the muscles can no longer be adequately controlled.

How do motor neuron diseases develop?

The causes vary and are sometimes not yet fully understood. Depending on the disease, genetic changes, disturbances in various cellular processes, and other neurological mechanisms can play a role.

One of the best-known motor neuron diseases is Amyotrophic Lateral Sclerosis (ALS). Other diseases can predominantly affect the upper or lower motor neurons.

What symptoms can occur?

Depending on which motor nerve cells are affected, different symptoms can arise:

• progressive muscle weakness

• muscle wasting (muscle atrophy)

• muscle twitching (fasciculations)

• muscle cramps

• increased muscle tension and spasticity

• difficulty walking and performing targeted movements

• impaired fine motor skills

• speech and swallowing disorders

• in advanced stages of the disease, impairment of respiratory muscles

Damage to upper and lower motor neurons

Damage to the upper motor neurons, which run from the brain to the spinal cord, can lead specifically to muscle stiffness, spasticity, and increased reflexes.

In contrast, damage to the lower motor neurons, which lead from the spinal cord or brainstem to the muscles, can primarily cause muscle weakness, muscle wasting, muscle twitching, and reduced reflexes.

Diagnosis and Treatment

Neurological evaluation can include, in addition to physical examination, electromyography (EMG), nerve conduction studies, MRI scans, as well as laboratory tests and, if necessary, genetic tests.

Treatment depends on the specific disease and its cause. Depending on the clinical picture, medications, physiotherapy, occupational therapy, speech therapy, respiratory therapy, and other supportive measures may be used.

New or progressive muscle weakness, muscle wasting, swallowing, or speech disorders should be neurologically evaluated.
