Rubella and Measles (Morbilli) – contagious viral infections with rash

Rubella and measles are two distinct viral infections. Both diseases can cause fever and a characteristic rash, but they differ in terms of pathogens, course of illness, and potential complications.

While measles is particularly significant due to potential severe respiratory and neurological complications, rubella during pregnancy poses a special danger to the unborn child.

Rubella

Rubella is caused by the rubella virus. The disease is often mild, especially in children, and can sometimes occur completely without noticeable symptoms.

Typical symptoms of rubella

Possible symptoms include:

• mild fever

• fatigue and malaise

• headaches

• runny nose

• mild sore throat

• fine-spotted pink rash

• swollen lymph nodes

Characteristic features include particularly enlarged lymph nodes:

• behind the ears

• in the neck

• at the back of the head

The rash often begins on the face and then spreads over the trunk and extremities.

Joint problems with rubella

In adolescents and adults, additional symptoms may include:

• joint pain

• joint stiffness

• temporary joint inflammation

These symptoms are more common, especially in adult women.

Rubella during pregnancy

A rubella infection during pregnancy can have particularly severe consequences.

The virus can cross the placenta and infect the unborn child. Particularly with an infection in early pregnancy, there is a high risk of severe developmental disorders.

The so-called congenital rubella syndrome can be associated with, among other things:

• hearing damage up to deafness

• eye damage

• cataracts

• congenital heart defects

• neurological developmental disorders

• growth disorders

Furthermore, an infection can lead to miscarriage or stillbirth.

Therefore, a suspected case of rubella or relevant contact during pregnancy must be medically clarified immediately.

Measles (Morbilli)

Measles is caused by the measles virus and is one of the most contagious infectious diseases in humans.

Transmission primarily occurs via infectious droplets and aerosols from the respiratory tract.

Typical symptoms of measles

The disease often begins with:

• high fever

• severe feeling of illness

• dry cough

• runny nose

• sore throat

• red and watery eyes

• light sensitivity

Characteristic Koplik spots may appear. These are small whitish spots on the oral mucosa.

Measles rash

After a few days, a pronounced rash typically develops.

This often begins:

• behind the ears

• at the hairline

• on the face

and then spreads over the neck, trunk, arms, and legs.

The red spots can increasingly merge. At the same time, the fever can rise significantly again.

Complications of measles

Measles is not a fundamentally harmless childhood disease.

Possible complications include:

• middle ear infection

• laryngitis and respiratory infections

• severe diarrhea

• fluid loss

• pneumonia

• bacterial secondary infections

• acute encephalitis

Severe courses can occur particularly in infants, adults, pregnant women, malnourished people, and individuals with weakened immune systems.

SSPE – rare late complication of measles

A rare but particularly severe late complication is subacute sclerosing panencephalitis (SSPE).

This is a progressive disease of the central nervous system that can occur years after the initial measles infection.

It leads to increasing neurological disorders and has a severe course.

Rubella or measles?

A reliable distinction based solely on the rash is not always possible.

Similar skin changes can also occur, for example, with:

• fifth disease

• scarlet fever

• roseola

• other viral infections

• drug reactions

In case of suspicion, targeted laboratory diagnostics may be necessary.

Infection and protection of others

Both measles and rubella can be transmitted even before the typical rash is fully visible.

Therefore, if one of these diseases is suspected, a doctor's office or medical facility should not be visited without prior announcement. Prior telephone contact enables appropriate protective and isolation measures.

Treatment

There is no routine specific antiviral treatment available for uncomplicated measles or rubella.

Treatment depends on symptoms and possible complications and may include:

• adequate fluid intake

• physical rest

• treatment of fever and pain

• medical monitoring in case of increased risk

• targeted treatment of complications that arise

Antibiotics are not effective against measles or rubella viruses. They are only used for corresponding additional bacterial infections.

Vaccination against measles and rubella

The most important prevention is vaccination against measles and rubella, usually as a combined measles-mumps-rubella (MMR) vaccine.

Complete vaccination significantly reduces the risk of illness and helps protect particularly vulnerable individuals.

Adequate rubella protection is particularly important for women planning a pregnancy.

The MMR vaccine is a live vaccine and is not administered during an existing pregnancy.
