Molluscum contagiosum – Viral skin infection caused by a poxvirus

Molluscum contagiosum, also known as water warts or molluscum, is a contagious, usually benign viral skin infection. It is caused by the Molluscum Contagiosum Virus (MCV) from the family of poxviruses (Poxviridae).

Molluscum contagiosum is particularly common in children, but can also affect adults. Typical symptoms are small, smooth skin nodules with a characteristic central dimple.

What do molluscum contagiosum lesions look like?

Typical features include:

• small, rounded nodules

• skin-colored, whitish, or slightly reddish

• smooth, shiny surface

• usually a few millimeters in size

• characteristic dimple in the center

• often multiple nodules simultaneously

• occasional itching

In the center, there may be a whitish, pasty mass containing numerous viral particles.

Where do molluscum contagiosum lesions commonly appear?

In children, molluscum lesions are often found on the:

• trunk

• arms

• legs

• armpit region

• backs of the knees

• elbow creases

• face

In adults, molluscum contagiosum can occur particularly in the:

• genital area

• lower abdomen

• groin area

• thigh area

Molluscum contagiosum in the genital area of adults can be transmitted through close or sexual skin contact.

How is molluscum contagiosum transmitted?

Transmission primarily occurs through direct skin-to-skin contact.

Transmission can also occur through shared objects, such as:

• towels

• washcloths

• clothing

• shaving equipment

• shared objects with close skin contact

Scratching or squeezing can spread the viruses to other skin areas. This often leads to the development of further molluscum lesions in the surrounding area, known as autoinoculation.

Molluscum Contagiosum in Eczema

People with atopic dermatitis (eczema) may be more frequently and sometimes more severely affected by molluscum contagiosum.

The impaired skin barrier and scratching facilitate the spread of viruses to adjacent skin areas.

Furthermore, an itchy eczema can develop around individual molluscum lesions.

Is molluscum contagiosum dangerous?

In otherwise healthy individuals, molluscum contagiosum is usually harmless and self-limiting.

The immune system can control the viral infection over time. The nodules often disappear within months, but can also persist for much longer.

After healing, no permanent skin changes usually occur, unless the areas have been severely scratched or secondarily infected.

Molluscum Contagiosum in Immunocompromised Individuals

In people with significantly weakened immune systems, molluscum contagiosum can:

• appear in unusually large numbers

• grow unusually large

• persist for a long time

• spread over larger skin areas

An unusually extensive infestation in adulthood may therefore require further medical investigation.

Diagnosis

The diagnosis can usually be made based on the characteristic appearance with the central indentation.

In cases of unclear skin changes, a dermatological examination and, rarely, a histological clarification may be necessary.

Treatment

Since molluscum contagiosum often disappears on its own in healthy individuals, active treatment is not always necessary.

If treatment is necessary or desired, various dermatological procedures may be considered depending on age, number, and location, for example:

• mechanical removal

• curettage

• cryotherapy (freezing)

• certain topically applied preparations

The appropriate method should be chosen individually, especially in children and for molluscum lesions on the face or genital area.

Do not squeeze or scratch

Molluscum contagiosum lesions should ideally not be squeezed or scratched. The contained material is infectious and can transmit the viruses to other skin areas.

Additionally, manipulation carries the risk of bacterial secondary infection and scarring.

Hygiene measures

To reduce spread, the following measures may be useful:

• wash hands thoroughly after touching

• do not scratch molluscum lesions

• do not share towels and washcloths

• avoid shaving over affected areas

• cover skin lesions as much as possible during close physical contact

• consistently care for the skin barrier in case of eczema
