Lymphedema – Lymphatic Congestion and Tissue Swelling

Lymphedema is a chronic swelling caused by a disruption of lymphatic drainage. In this condition, lymph fluid cannot be adequately transported away by the lymphatic vessels and accumulates in the tissues.

Arms or legs are most commonly affected. However, lymphedema can also occur in other body regions.

How does lymphedema develop?

The lymphatic system transports excess tissue fluid, proteins, and other components from the tissues back into the bloodstream. If lymphatic vessels or lymph nodes are underdeveloped, damaged, or have been removed, lymphatic congestion can occur.

Two main forms are distinguished:

  • Primary lymphedema: arises from a congenital or genetically determined malformation of the lymphatic system and can appear in childhood or later.

  • Secondary lymphedema: arises from damage to a previously normally developed lymphatic system, for example, after operations, removal of lymph nodes, radiation therapy, injuries, infections, or certain tumor diseases.

What symptoms can occur?

Typical complaints include:

  • swelling of an arm or leg

  • sensation of tension and heaviness

  • feeling of pressure in the affected area

  • restricted mobility

  • initially soft, later partially hardened swelling

  • changes and thickening of the skin

  • increased susceptibility to skin infections

Initially, the swelling may partially subside after rest or elevation. In long-standing lymphedema, permanent tissue changes can develop.

Diagnosis and Treatment

Diagnosis is based on symptoms and a physical examination. If necessary, additional examinations of the lymphatic and vascular systems can be performed.

An important component of treatment is complex physical decongestive therapy (CPDT). It may include:

  • manual lymphatic drainage

  • compression therapy

  • movement and targeted exercises

  • skin care

  • instruction in self-management

The goal is to reduce swelling, support lymphatic drainage, and prevent complications as much as possible.

Important note

New or suddenly increasing unilateral swelling should be medically evaluated, as other causes such as venous thrombosis, infection, or other vascular diseases may also be possible in addition to lymphedema.
