Osteonecrosis of the Femoral Head / Neck – Avascular Necrosis of the Femoral Head

Osteonecrosis means that bone tissue dies due to insufficient blood supply. In the hip area, it is usually femoral head necrosis (osteonecrosis of the femoral head), also known as avascular or aseptic osteonecrosis.

The femoral head at the upper end of the thigh bone (femur) is primarily affected. Without treatment, the bone structure can become progressively weakened, and the femoral head may partially collapse. This can lead to severe hip osteoarthritis.

How does femoral head necrosis develop?

The disease develops due to impaired blood supply to the bone tissue. Possible causes and risk factors include:

• long-term or high-dose corticosteroid therapy

• high alcohol consumption

• injuries, especially hip dislocations or femoral neck fractures

• certain blood and clotting disorders

• sickle cell disease

• autoimmune diseases such as lupus erythematosus

• certain metabolic disorders

• after organ transplants

• rarely after radiation or certain other therapies

In some patients, the cause remains unknown (idiopathic osteonecrosis).

What symptoms can occur?

In the early stage, the disease may initially be asymptomatic. Later, the following may occur:

• groin pain

• hip pain

• buttock or thigh pain

• pain during exertion

• later also rest and night pain

• restricted hip mobility

• pain when walking

• increasing limping

The symptoms can sometimes also radiate to the knee.

Consider cortisone as a risk factor

Long-term or high-dose treatment with corticosteroids is a known risk factor for femoral head necrosis.

However, this does not mean that every patient undergoing cortisone therapy develops osteonecrosis. In cases of corresponding hip or groin pain, this connection should be considered during the medical examination.

Diagnosis – MRI is particularly important

In the early stage, a normal X-ray may still be unremarkable. Magnetic resonance imaging (MRI) is particularly sensitive and can often detect femoral head necrosis early.

Diagnostics may include:

• orthopedic examination

• X-rays

• hip MRI

• CT if necessary

• examinations for possible underlying diseases and risk factors

An early diagnosis is important because treatment options depend on the stage of the disease.

Treatment

Therapy depends on the stage, size, and location of the necrosis, as well as the patient's age and symptoms.

Possible measures include:

• adjustment of load

• pain management

• treatment or reduction of modifiable risk factors

• physiotherapeutic measures

• joint-preserving surgical procedures, especially in early stages

• e.g., core decompression (bone decompression drilling) in selected patients

• in cases of advanced destruction of the femoral head, possibly artificial hip joint replacement

The goal of early treatment is particularly to prevent or delay the collapse of the femoral head as much as possible.

Homeopathy according to Dr. Banerji

In homeopathy according to Dr. Banerji, specific homeopathic remedies and potencies are used according to the Banerji protocols for various bone, joint, and movement complaints.

The restoration of already dead bone tissue, restoration of disturbed blood supply, or prevention of femoral head collapse through homeopathic remedies is not scientifically proven. Necessary orthopedic diagnostics and treatment should therefore not be replaced or delayed.

Important Note

New or increasing groin and hip pain, especially with existing or previous corticosteroid therapy, should be clarified by an orthopedic specialist. In case of suspected femoral head necrosis, an early MRI examination can be crucial.
