Crohn's Disease – Chronic Inflammatory Bowel Disease

Crohn's disease is a chronic inflammatory disease of the digestive tract and, together with ulcerative colitis, belongs to the group of chronic inflammatory bowel diseases (IBD).

The inflammation can fundamentally affect the entire digestive tract from the mouth to the anus. However, the last section of the small intestine (terminal ileum) and the large intestine are particularly often affected.

In contrast to ulcerative colitis, inflammation in Crohn's disease often occurs in sections and can affect deeper layers of the intestinal wall.

What symptoms can occur?

Typical symptoms include:

  • recurrent or chronic diarrhea

  • abdominal pain and intestinal cramps

  • frequent pain in the lower right abdomen

  • weight loss

  • loss of appetite

  • fatigue and exhaustion

  • fever during active inflammatory flare-ups

  • nausea

  • sometimes blood or mucus in the stool

The disease often progresses in flare-ups with active inflammatory phases and periods with few or no symptoms.

Fistulas and Abscesses

Since inflammation in Crohn's disease can affect the entire intestinal wall, fistulas and abscesses can develop.

Possibilities include:

  • anal fistulas

  • perianal abscesses

  • fistulas between different sections of the intestine

  • fistulas between the intestine and skin

  • rarely connections to other organs

Recurrent anal fistulas or anal abscesses can therefore be an indication of Crohn's disease.

Intestinal strictures

Chronic inflammation can lead to scarring and strictures (narrowing of the intestine).

Possible symptoms include:

  • cramping abdominal pain

  • feeling of fullness

  • bloated abdomen

  • nausea and vomiting

  • difficulty with bowel movements

A complete bowel obstruction (ileus) is a medical emergency.

Symptoms outside the intestine

Crohn's disease can also affect other organs. Possibilities include:

  • joint inflammation

  • skin changes

  • eye inflammation

  • liver and bile duct diseases

  • aphthous ulcers in the mouth

  • kidney stones

  • gallstones

Nutrient deficiency

With long-standing or severe disease, nutrients may be absorbed less effectively.

In particular, these may include:

  • iron deficiency and anemia

  • vitamin B12 deficiency, especially with involvement or removal of the terminal ileum

  • folic acid deficiency

  • vitamin D deficiency

  • weight and muscle loss

Diagnosis

The following can be used for diagnosis:

  • blood count and inflammatory markers

  • iron, vitamin, and other nutrient levels

  • stool test, especially calprotectin

  • exclusion of infectious bowel diseases

  • colonoscopy with examination of the terminal ileum

  • tissue samples (biopsies)

  • ultrasound of the bowel

  • MRI or MR enterography

  • for specific questions, further imaging examinations

Treatment

The goal of treatment is to control inflammation, reduce symptoms, achieve the longest possible remission, and prevent complications.

Depending on disease activity and affected areas, the following may be used:

  • corticosteroids to treat certain acute flare-ups

  • immunosuppressants

  • biologics

  • other targeted immunotherapies

  • nutritional therapy in certain situations

  • treatment of vitamin and mineral deficiencies

In cases of fistulas, abscesses, pronounced strictures, or other complications, endoscopic or surgical interventions may be necessary.

Surgery can eliminate complications, but it does not fundamentally cure Crohn's disease, as the disease can recur in other sections of the intestine later on.

Note on smoking

Smoking has a particularly unfavorable effect on Crohn's disease and can increase the risk of flare-ups, complications, and surgeries. Therefore, quitting smoking is an important part of the treatment.

In homeopathy according to Dr. Banerji, specific homeopathic remedies and potencies are used according to the Banerji protocols for various inflammatory diseases and digestive tract complaints.

Reliable control of chronic bowel inflammation, treatment of fistulas or abscesses, or prevention of structural bowel damage through homeopathic remedies is not scientifically proven. Therefore, necessary gastroenterological or immunomodulatory treatment should not be replaced or discontinued independently.

Important Note

Severe or increasing abdominal pain, a severely bloated abdomen, persistent vomiting, high fever, larger amounts of blood in the stool, or absence of stool and flatulence can indicate a severe complication such as an abscess, bowel stricture, or bowel obstruction and must be medically clarified quickly.
