Liver cysts are fluid-filled cavities in the liver tissue. Simple liver cysts are relatively common, usually benign, and are often discovered incidentally during an ultrasound, CT, or MRI examination.
Many liver cysts cause no symptoms and require no treatment. However, it is important to differentiate between a simple liver cyst and other cystic changes in the liver.
What types are there?
Possible cystic changes include:

congenital multiple liver cysts

polycystic liver disease

parasitic cysts, for example, due to echinococci

rarely cystic tumors or other cystic neoplasms
What symptoms can occur?
Small simple liver cysts are usually asymptomatic.
Larger cysts can cause discomfort by pressing on surrounding structures:

early satiety

palpable mass with very large cysts

rarely nausea
In exceptional cases, very large cysts can displace adjacent organs or bile ducts.
Simple Liver Cyst
A typical simple liver cyst has characteristic features on ultrasound. If the findings are clear and there are no symptoms, no therapy is often required.
Not every liver cyst needs to be monitored regularly. Whether follow-up checks are useful depends on the imaging findings and the individual situation.
When is a more detailed clarification necessary?
Further investigation may be necessary, especially for:

unusual or irregular cyst wall

septae or subdivisions within the cyst

solid components or nodules

rapid increase in size
-
pain or fever
-
several pronounced cysts

unclear diagnosis

suspicion of a parasitic cyst
Depending on the findings, CT or MRI can be used for more precise evaluation.
Echinococcal Cyst
An important, albeit rarer, cause is infection with Echinococcus. This can lead to parasitic cysts, particularly in the liver.
If an echinococcal cyst is suspected, it must not be uncritically punctured, as this can lead to severe complications. Treatment belongs in specialized medical care.
Treatment
Asymptomatic simple liver cysts often do not require treatment.
For large or symptomatic cysts, interventional or surgical procedures may be used depending on the findings.
The therapy always depends on the type, size, location, and cause of the cyst.
