Hypogonadism, Cryptorchidism, and Gynecomastia – Deficiency of Male Sex Hormones

Male hypogonadism is a hormonal disorder in which the testicles do not produce enough testosterone and/or sperm production is impaired. The condition can be congenital or develop later in life.

A testosterone deficiency can affect pubertal development, sexual function, fertility, musculature, bones, and general well-being.

How does male hypogonadism develop?

Broadly, two forms are distinguished:

• Primary hypogonadism: The cause lies directly in the testicles. Despite appropriate hormonal stimulation, they do not produce enough testosterone.

• Secondary hypogonadism: The disorder lies in the pituitary or hypothalamus. As a result, the testicles are not sufficiently stimulated by the hormones LH and FSH.

Possible causes include genetic or congenital diseases, testicular damage, inflammation, certain medications, pituitary diseases, and various chronic conditions.

What symptoms can testosterone deficiency cause?

Depending on age and severity, the following may occur:

• decreased libido

• erectile dysfunction

• reduced sperm production and impaired fertility

• decrease in muscle mass and strength

• increase in body fat percentage

• reduced beard and body hair growth

• fatigue and reduced performance

• decrease in bone density leading to osteoporosis

• delayed or incomplete pubertal development in adolescents

• partial gynecomastia

Cryptorchidism – Undescended Testicle

Cryptorchidism (undescended testicle) is a developmental disorder in which one or both testicles do not remain in the scrotum after birth.

Untreated cryptorchidism can impair future fertility and is associated with an increased risk of testicular cancer. Therefore, early pediatric or urological assessment and treatment are important.

Gynecomastia – Enlargement of the Male Breast Gland

Gynecomastia refers to a benign enlargement of male breast glandular tissue. The cause is often an altered ratio between the effects of androgens and estrogens.

Gynecomastia can occur, among other things, during puberty, in old age, with hypogonadism, and in connection with certain medications or liver, kidney, and hormonal disorders.

It must be distinguished from pseudogynecomastia, in which primarily increased fatty tissue is present in the breast area.

Diagnosis

If hypogonadism is suspected, the following hormone levels, in particular, can be examined:

• Testosterone

• LH and FSH

• SHBG

• possibly prolactin and other hormones

The diagnosis of testosterone deficiency should not be based solely on symptoms but confirmed by appropriate morning hormone measurements. Depending on the findings, further endocrinological, urological, or imaging examinations may be necessary.

Treatment

Treatment depends on the cause. In the case of confirmed testosterone deficiency, testosterone replacement therapy may be considered under certain conditions. If there is a desire to have children, the therapy must be chosen particularly carefully, as external testosterone intake can reduce the body's own sperm production.

Cryptorchidism and new-onset or pronounced gynecomastia require independent medical clarification and treatment.
