Hyperthyroidism – Overactive Thyroid

Hyperthyroidism, also known as an overactive thyroid, is a condition where there are too many thyroid hormones in the body. This accelerates numerous metabolic processes and particularly affects the heart, circulation, nervous system, and energy balance.

The thyroid gland primarily produces the hormones thyroxine (T4) and triiodothyronine (T3). In the case of hyperthyroidism, their concentrations or their effects in the body are elevated.

How does an overactive thyroid develop?

The most common causes include:

• Graves' disease: an autoimmune disease in which antibodies stimulate the thyroid gland to produce more hormones

• Thyroid autonomy: individual nodules or larger areas of the thyroid gland produce thyroid hormones independently of the normal regulatory cycle

• Thyroid inflammation (thyroiditis): stored hormones can be temporarily released in increased amounts

• overdosing of thyroid hormones

• certain medications or a high iodine intake in individuals with a predisposition

What symptoms can occur?

Typical symptoms include:

• racing heart and palpitations

• cardiac arrhythmias, especially atrial fibrillation

• inner restlessness and nervousness

• trembling hands

• increased sweating and heat intolerance

• weight loss despite normal or increased appetite

• muscle weakness

• sleep disturbances

• more frequent bowel movements or diarrhea

• fatigue and exhaustion

• menstrual disorders

• hair loss

• partial enlargement of the thyroid gland (goiter)

Graves' Disease and Eyes

In Graves' disease, additional eye changes can occur. This so-called endocrine orbitopathy can lead to dry or irritated eyes, a feeling of pressure, bulging eyes, light sensitivity, or double vision, among other things.

Diagnosis

To clarify, thyroid hormone levels are determined:

• TSH

• free T4 (fT4)

• free T3 (fT3)

Depending on the suspected cause, thyroid antibodies, ultrasound examinations, and a thyroid scintigraphy may also be necessary.

Treatment

Treatment depends on the cause and extent of the overactivity. Depending on the disease, thyreostatics, radioiodine therapy, or surgery may be considered. Accompanying symptoms such as severe heart palpitations can also be treated.

Thyroid Storm

A very severe, suddenly uncontrolled hyperthyroidism can lead to a thyroid storm. This can cause high fever, extreme racing heart, cardiac arrhythmias, vomiting, diarrhea, restlessness, confusion, and impaired consciousness. This is a medical emergency.
