Jaundice – Yellowing of skin and eyes due to elevated bilirubin

Jaundice (icterus) is a yellowing of the skin, mucous membranes, and particularly the whites of the eyes (sclera). It is caused by an elevated concentration of the bile pigment bilirubin in the blood.

Jaundice is not a disease in itself, but rather a symptom. The underlying cause should be medically investigated, as it can be due to diseases of the liver and bile ducts, as well as changes in the blood.

How does jaundice develop?

Bilirubin is primarily formed during the breakdown of red blood cells. It is transported via the blood to the liver, processed there, and then excreted into the intestines via bile.

Jaundice can occur if this process is disrupted at different points.

Important causes include:

• increased breakdown of red blood cells (hemolysis)

• acute or chronic hepatitis

• liver cirrhosis

• alcoholic or drug-induced toxic liver damage

• gallstones in the common bile duct

• cholangitis

• narrowing of the bile ducts

• diseases or tumors of the bile ducts

• diseases or tumors of the pancreas

• congenital disorders of bilirubin metabolism, such as Gilbert's syndrome

What additional symptoms can occur?

Depending on the cause, these may include:

• yellow skin and eyes

• dark urine

• light to clay-colored stools

• itching

• fatigue and weakness

• loss of appetite

• nausea

• upper abdominal pain

• fever or chills

• weight loss

Jaundice due to bile duct obstruction

If the flow of bile is blocked, it is referred to as cholestatic or posthepatic jaundice.

Possible causes include:

• gallstone in the common bile duct

• inflammation or narrowing of the bile ducts

• tumors of the bile ducts

• pancreatic tumors

• other mechanical obstructions to flow

Typically, there may be jaundice, dark urine, pale stools, and severe itching.

Jaundice in liver diseases

In hepatitis or other liver damage, the liver may no longer be able to adequately process and excrete bilirubin.

Possible causes include, for example:

• viral hepatitis

• autoimmune hepatitis

• alcohol-related liver damage

• medication-induced or toxic hepatitis

• advanced liver cirrhosis

Diagnosis – It is essential to clarify the cause

New onset jaundice should be medically investigated. Diagnostics may include:

• total and direct bilirubin

• liver enzymes

• alkaline phosphatase and gamma-GT

• complete blood count

• coagulation values

• hepatitis tests

• ultrasound of the liver, gallbladder, and bile ducts

• depending on findings, CT or MRI/MRCP

• ERCP if necessary

Treatment

Jaundice itself is not treated in isolation. The treatment of the underlying cause is crucial.

For example, in the case of bile duct obstruction, an endoscopic treatment using ERCP may be necessary. For hepatitis, therapy depends on the specific cause.
