Prolactinoma – prolactin-producing pituitary adenoma
A prolactinoma is a usually benign tumor of the pituitary gland that produces an increased amount of the hormone prolactin. This leads to an elevated prolactin level in the blood, known as hyperprolactinemia.
The pituitary gland is located at the base of the brain and controls numerous bodily functions through various hormones. Prolactin is particularly important for breast development and milk production.
How does a prolactinoma develop?
A prolactinoma develops due to an increase in prolactin-producing cells in the pituitary gland. In most cases, the reason why these cells change is not clearly known.
Depending on their size, they are primarily distinguished as:

Microprolactinoma: smaller than 10 mm

Macroprolactinoma: 10 mm or larger
Larger prolactinomas can also cause symptoms by pressing on adjacent structures.
What are the effects of an elevated prolactin level?
Too much prolactin can affect the secretion of sex hormones and thus cause different symptoms in women and men.
In women, symptoms may include:

disturbances of other pituitary hormones
A restriction of the lateral visual field can be particularly characteristic if the tumor presses on the optic chiasm.
Diagnosis
To clarify, the prolactin level in the blood is first determined. Since medications, pregnancy, hypothyroidism, and other diseases can also increase prolactin levels, these causes must be considered.
If there is a corresponding suspicion, the pituitary gland is examined using magnetic resonance imaging (MRI). Additionally, other pituitary hormones and, if necessary, the visual field can be checked.
Treatment
Prolactinomas can often be treated very well with medication. Dopamine agonists, such as cabergoline, are primarily used. These can lower prolactin levels and often significantly reduce the size of the tumor.
Surgery is an option particularly if medications are not sufficiently effective, are not tolerated, or if there are specific medical reasons for it.
