Cardiac Arrhythmias – see also Atrial Fibrillation (AFib)
Cardiac arrhythmias are disturbances in the normal electrical impulse formation or conduction of the heart. The heart can beat too fast, too slow, or irregularly.
Cardiac arrhythmias range from harmless extra beats to rhythm disturbances requiring treatment or being acutely life-threatening.
How do cardiac arrhythmias develop?
The normal heartbeat is controlled by electrical impulses that originate in the sinoatrial node, the heart's natural pacemaker, and spread throughout the heart's specialized conduction system.
If the generation or transmission of these electrical signals is disrupted, an arrhythmia can occur.
Possible causes and contributing factors include:
-
Coronary artery disease
-
Heart attack or previous heart muscle damage
-
Heart valve diseases
-
Heart failure
-
High blood pressure
-
Thyroid dysfunction
-
Electrolyte imbalances, such as potassium or magnesium
-
Certain medications
-
Alcohol and other stimulating substances
-
Congenital disorders of the conduction system
What types of cardiac arrhythmias are there?
More common or medically significant types include:
-
Extrasystoles: additional heartbeats often perceived as a fluttering or skipped beat
-
Tachycardia: abnormally fast heartbeat
-
Bradycardia: abnormally slow heartbeat
-
Atrial fibrillation (AFib): rapid and disorganized electrical activity in the heart's atria
-
Atrial flutter: rapid, usually more regularly organized activity of the atria
-
Ventricular tachycardia: rapid rhythm disturbance originating in the heart's ventricles
-
Ventricular fibrillation: disorganized electrical activity of the heart's ventricles, an immediate life-threatening condition
Atrial Fibrillation (AFib)
Atrial fibrillation is one of the most common sustained cardiac arrhythmias. In AFib, the atria no longer beat regularly but are activated electrically very rapidly and in a disorganized manner.
Atrial fibrillation can cause heart palpitations, a fluttering heart, shortness of breath, dizziness, or reduced exercise capacity, but can also go completely unnoticed.
A significant potential complication is the formation of blood clots in the heart. If such a clot travels to a blood vessel in the brain, it can cause an ischemic stroke.
What symptoms can occur?
Depending on the type of arrhythmia, possible symptoms include:
-
Racing heart or palpitations
-
Fluttering or skipped heartbeats
-
Irregular pulse
-
Dizziness
-
Weakness and reduced exercise tolerance
-
Shortness of breath
-
Pressure or pain in the chest
-
Brief loss of consciousness (syncope)
Some cardiac arrhythmias cause no noticeable symptoms.
Diagnosis and Treatment
Diagnosis typically involves ECG, Holter monitoring, and potentially further cardiological examinations.
Treatment depends on the type and cause of the rhythm disturbance. Depending on the findings, medications, electrical cardioversion, catheter ablation, pacemakers, or other procedures may be used. For atrial fibrillation, blood thinning (anticoagulation) may also be necessary to prevent strokes.
Important Note
New onset of severe heart palpitations or a fluttering heart should be medically evaluated. If chest pain, severe shortness of breath, circulatory problems, or loss of consciousness occur simultaneously, immediate medical attention is required.
