Herpes – Viral infection caused by herpes simplex viruses (HSV)

Herpes simplex is a widespread viral infection caused by herpes simplex viruses (HSV). It is characterized by recurrent, clustered, painful or burning blisters on the skin and mucous membranes.

After the initial infection, the viruses remain lifelong in certain nerve cells of the body. They can stay inactive for a long time and be reactivated later. This is why herpes can recur in some people.

Herpes simplex virus type 1 and type 2

Mainly distinguished are:

• HSV-1: often the cause of cold sores and infections in the mouth and facial area

• HSV-2: often associated with genital herpes

However, this classification is not absolute. Both virus types can infect both the oral and genital areas.

Cold Sores – Herpes labialis

Cold sores are among the most common manifestations of an HSV infection.

Typical symptoms include:

• Tingling

• Burning

• Itching

• Feeling of tension

• Small clustered blisters

• Painful skin areas

• Later crust formation

Often, a new outbreak announces itself with tingling or burning before visible blisters appear.

Genital Herpes – Herpes genitalis

Herpes in the genital area can cause, among other things:

• Painful blisters

• Small ulcers after blisters burst

• Burning and itching

• Pain during urination

• Swollen lymph nodes

• In case of primary infection, sometimes fever and pronounced malaise

Genital herpes is a sexually transmitted infection.

How is herpes transmitted?

Transmission occurs through direct contact with infected skin or mucous membranes or with infectious secretions.

Transmission is particularly likely with visible blisters and open skin lesions. However, HSV can also be shed and transmitted without visible symptoms.

What can trigger a new herpes outbreak?

Reactivation can be favored by, among other things:

• Infections and fever

• Strong sun or UV radiation

• Physical exertion

• Stress

• Lack of sleep

• Hormonal changes

• Weakened immune system

• Injuries or irritations to the affected skin region

Individual triggers can vary greatly.

Herpes is not the same as herpes zoster

Herpes simplex should not be confused with Herpes zoster (shingles).

Shingles is caused by the varicella-zoster virus – the same virus that causes chickenpox. It typically presents as a painful, often unilateral rash limited to a specific nerve area.

Diagnosis

In cases with typical appearance, diagnosis can often be made clinically.

For unclear, severe, or atypical courses, direct virus detection, for example, via PCR from a swab of the affected area, can be performed.

Treatment

Antiviral drugs can be used for treatment, for example:

• Aciclovir

• Valaciclovir

• Famciclovir

For cold sores, an early start of local or – depending on the situation – systemic antiviral treatment can influence the course.

For pronounced or frequently recurring genital herpes, long-term antiviral suppressive therapy may be considered by a doctor.

Preventing infection

During an active herpes outbreak, special attention should be paid to hygiene.

Among other things, it is advisable to:

• Avoid touching blisters as much as possible

• Wash hands thoroughly after contact

• Do not share lip care products

• Avoid kissing during active cold sores

• Avoid oral sex during cold sores

• Avoid sexual contact during active genital herpes

Herpes in the eye

A herpes simplex infection of the eye can particularly affect the cornea (herpes keratitis) and endanger vision.

In case of a red or painful eye, light sensitivity, or worsening vision in connection with herpes, a rapid ophthalmological examination is necessary.

Herpes in newborns and immunocompromised individuals

In newborns and people with severely weakened immune systems, HSV infections can be severe and spread to internal organs or the nervous system.

A herpes infection during pregnancy or genital herpes around the time of birth should be discussed with the treating physician.
