Acute Sciatica – Lower Back Pain Radiating to the Leg

Sciatica refers to pain caused by irritation or compression of nerve roots in the lumbar spine (LWS), typically radiating from the lower back or buttocks into the leg.

If significant pain is simultaneously present in the lumbar spine area, it is often referred to as lumbosciatica.

How does acute sciatica occur?

Often, irritation or narrowing of a nerve root is the underlying cause. Possible causes include:

• Herniated disc (disc herniation)

• Disc bulging

• Degenerative changes in the lumbar spine

• Spinal canal stenosis

• Narrowing of the nerve exit openings

• Spondylolisthesis

• Less commonly, injuries, inflammations, or other diseases

However, not every acute back pain is sciatica.

What are the typical symptoms?

Possible symptoms include:

• Acute pain in the lower back

• Buttock pain

• Radiation into the thigh, lower leg, or foot

• Stabbing, burning, or electric shock-like pain

• Tingling or crawling sensation

• Numbness

• Restricted mobility

• Pain when coughing, sneezing, or straining

• Muscle weakness or paralysis in cases of more severe nerve compression

The exact radiation depends on which nerve root is affected.

Acute Sciatica and Herniated Disc

A lumbar disc herniation is a common cause of acute sciatica. In this case, extruded disc tissue can irritate or compress a nerve root.

Many disc herniations and sciatica symptoms can be treated without surgery.

Diagnosis

The diagnosis is initially made based on the symptoms and an orthopedic or neurological examination. This includes checking muscle strength, reflexes, and sensation.

Magnetic resonance imaging (MRI) is not immediately necessary for every uncomplicated acute sciatica. Imaging may be required in cases of neurological deficits, warning signs, or persistent severe symptoms.

Treatment

For uncomplicated acute sciatica, pain reduction and maintaining mobility are the primary goals.

Depending on the findings, the following may be used:

• Adapted physical activity

• Pain and anti-inflammatory medications

• Heat for accompanying muscular symptoms

• Physiotherapy

• Later, targeted muscle building and movement training

Prolonged complete bed rest is generally not recommended. However, movement should be adapted to the severity of the symptoms.

Cauda Equina Syndrome – Emergency Attention

In rare cases, a large disc herniation can severely compress several nerves in the lower spinal canal simultaneously.

Warning signs include:

• New problems with urination or urinary retention

• Loss of bladder or bowel control

• Numbness in the genital, perineal, or gluteal area

• Suddenly increasing weakness or paralysis of one or both legs

These symptoms may indicate Cauda Equina Syndrome and require immediate medical or neurosurgical clarification.

Reliable elimination of mechanical nerve compression or treatment of neurological deficits through homeopathic remedies is not scientifically proven. Necessary orthopedic, neurological, or neurosurgical treatment should therefore not be replaced or delayed.
