Hirsutism in women refers to excessive hair growth in a typical male distribution pattern. This involves the growth of strong, dark hairs, particularly on the upper lip, chin, chest, abdomen, back, or thighs.

Virilization goes beyond this. It results from a more pronounced effect of male sex hormones (androgens) and can cause additional masculinizing symptoms in addition to increased body hair.

How does hirsutism develop?

Hirsutism can result from elevated androgen levels or increased sensitivity of hair follicles to androgens.

Possible causes include:

• Polycystic Ovary Syndrome (PCOS)

• genetic or familial predisposition

• adrenal gland disorders

• congenital disorders of steroid hormone synthesis, such as congenital adrenal hyperplasia

A rapid onset or significantly increasing virilization should be thoroughly investigated.

Diagnosis

In addition to a clinical examination, various hormone levels can be determined for clarification. These include, depending on the situation, for example, testosterone, DHEAS, androstenedione, and 17-hydroxyprogesterone.

In case of abnormal findings, further hormonal investigations as well as ultrasound or other imaging procedures may be necessary.

Treatment

Treatment depends on the cause. In the case of an underlying hormonal disorder, its targeted treatment is the priority. Additionally, for hirsutism, depending on the situation, hormonal or anti-androgenic therapies, as well as cosmetic hair removal procedures, can be used.
