Anal itching (Pruritus ani) – with or without hemorrhoids

Anal itching (Pruritus ani) describes an unpleasant, sometimes very severe itching in the area of the anus and the surrounding skin. It can be temporary or chronic and may worsen, especially at night or after a bowel movement.

Hemorrhoids can be a cause or a predisposing factor. However, anal itching can also occur without hemorrhoids and have numerous other causes.

Typical Symptoms

In addition to the actual itching, the following may occur:

• Burning

• Reddened or irritated skin

• Weeping

• Soreness

• Small skin tears

• Scratch marks

• Pain

• Feeling of a foreign body

• Moisture in the anal area

• Traces of blood on toilet paper

Excessive or repeated scratching can create an itch-scratch cycle. The skin becomes increasingly irritated and thus begins to itch even more intensely.

Anal itching due to hemorrhoids

Enlarged hemorrhoids can cause small amounts of mucus or stool to leak from the anal canal, irritating the sensitive skin around the anus.

Signs of hemorrhoidal disease may include:

• Itching and burning

• Weeping

• Bright red blood during bowel movements

• Feeling of a foreign body or pressure

• Palpable or protruding lumps

Female worms prefer to lay their eggs at night in the anal region, which can cause characteristic severe itching.

Skin irritation due to excessive hygiene

Out of concern for poor hygiene, the anal area is often cleaned particularly intensively when itching occurs. However, this can worsen the symptoms.

Soap, perfumed wet wipes, disinfectants, and frequent washing can damage the natural skin barrier and cause an irritant anal eczema.

In cases of persistent or recurrent anal itching, the cause should be clarified.

• Swab in case of suspected fungal or bacterial infections

• Examination for pinworms

Depending on the findings, the following may be helpful:

• Gentle anal hygiene

• Avoiding perfumed wet wipes and aggressive cleaning agents

• Careful but gentle drying

• Avoiding scratching

• Regulating stool consistency

• Treatment of hemorrhoidal disease

• Treatment of eczema or contact allergies

• Targeted treatment of fungal, parasitic or other infections

• Treatment of an existing anal fissure

For hemorrhoids, treatment depends on their severity and ranges from conservative measures to various proctological procedures.

A reliable treatment of hemorrhoids, infections, parasites, or other causes of anal itching with homeopathic remedies is not scientifically proven.

Therefore, with persistent symptoms, the underlying cause should be medically or proctologically clarified. Necessary treatment should not be replaced or delayed.

Important Note

Persistent or recurrent anal itching should be medically or proctologically examined, especially in cases of blood in the stool, severe pain, palpable changes, purulent or unusual discharge, non-healing skin areas, weight loss, or altered bowel habits.

Rectal bleeding should not automatically be attributed to hemorrhoids.
