Gout – Excess Uric Acid and Uric Acid Crystal Deposition

Gout (arthritis urica) is a metabolic disorder where uric acid crystals can deposit in joints and other tissues. This usually requires a prolonged elevation of uric acid levels in the blood, known as hyperuricemia.

Not everyone with elevated uric acid levels develops gout. However, if monosodium urate crystals form and deposit, they can trigger strong inflammatory reactions and typical gout attacks.

How does excess uric acid occur?

Uric acid is formed during the breakdown of purines. These occur naturally in the body and are also absorbed through certain foods. Uric acid is predominantly excreted by the kidneys.

Hyperuricemia usually develops when the kidneys do not excrete enough uric acid. Less commonly, too much uric acid is produced in the body.

Possible contributing factors include:

• genetic predisposition

• impaired kidney function

• purine-rich diet

• high alcohol consumption

• obesity and metabolic syndrome

• fasting or rapid weight loss

• certain medications, such as some diuretics

• diseases with increased cell breakdown

The Acute Gout Attack

An acute gout attack often begins suddenly, frequently at night. Inflammation of the metatarsophalangeal joint of the big toe (podagra) is particularly typical. However, other joints can also be affected.

Typical symptoms include:

• sudden onset of severe joint pain

• pronounced redness and warmth

• joint swelling

• extreme sensitivity to touch and pressure

• restricted mobility

• occasionally fever and general malaise

Chronic Gout

If uric acid levels remain elevated long-term, recurrent gout attacks can occur. In the further course, larger crystal deposits, known as gouty tophi, can form.

Chronic gout can permanently damage joints. Uric acid can also contribute to the formation of certain kidney stones and stress the kidneys.

Diagnosis and Treatment

To clarify the condition, among other things, the uric acid level in the blood is determined. During an acute gout attack, however, the uric acid level can also temporarily be within the normal range. In cases of unclear joint inflammation, an examination of the joint fluid for uric acid crystals may be necessary.

Treatment includes therapy for the acute gout attack and, if indicated, a sustained reduction of uric acid levels. Additionally, dietary and lifestyle measures can be beneficial.
