Submucosal cysts of the epiglottis, also known as epiglottic cysts, are usually benign, fluid or secretion-filled cavities that form beneath the mucous membrane of the epiglottis.

The epiglottis is located at the entrance to the larynx and helps protect the airways during swallowing. Small cysts may go unnoticed and be discovered incidentally during an examination. Larger cysts, however, can cause swallowing difficulties or discomfort in the airway area.

How do epiglottic cysts develop?

Often, these are so-called retention cysts. In this case, the excretory duct of a small mucous gland becomes blocked, causing secretions to accumulate under the mucous membrane and form a cyst.

Possible causes or contributing factors include:

  • Blockage of mucous gland ducts

  • chronic irritation or inflammation

  • changes after injuries or procedures

  • rarely, congenital changes

What symptoms can occur?

Small cysts often cause no symptoms. Depending on their size and location, the following may occur:

  • feeling of a foreign body or lump in the throat

  • difficulty swallowing

  • constant throat clearing

  • cough reflex

  • hoarseness or voice changes

  • discomfort when swallowing food

  • in severe cases, breathing difficulties

Are epiglottic cysts dangerous?

Most epiglottic cysts are benign. However, size and location are crucial. A larger cyst can constrict the area of the laryngeal entrance and, in rare cases, impair breathing.

Furthermore, changes in this area should be examined by a specialist to rule out other diseases or masses.

Diagnosis and Treatment

Diagnosis is usually made by an ENT examination of the larynx, for example, by flexible or rigid laryngoscopy.

Small, asymptomatic cysts can be monitored depending on the findings. For larger or symptomatic cysts, endoscopic surgical removal or opening of the cyst may be necessary.
