Endometriosis is a chronic, benign disease in which endometrial-like tissue is found outside the uterine cavity. These so-called endometriotic lesions can trigger inflammatory reactions and lead to pain, adhesions, and in some cases, fertility problems.

The ovaries, fallopian tubes, peritoneum, and the area behind the uterus are frequently affected. However, endometriosis can also occur in other organs.

How does endometriosis develop?

The exact cause of endometriosis is not yet fully understood. It is presumed that several factors interact. These include:

• retrograde menstruation, where menstrual blood flows back through the fallopian tubes into the abdominal cavity

• hormonal influences, especially estrogen

• genetic factors

• changes in the immune system

• inflammatory processes

• changes in certain cells and tissues

Endometriotic lesions often react to hormonal changes during the menstrual cycle and can thus cause recurrent inflammation and pain.

What symptoms can occur?

The symptoms vary greatly from person to person. Some affected individuals have extensive endometriosis without severe symptoms, while others develop significant pain even with smaller lesions.

Typical symptoms include:

• severe or increasingly painful menstruation

• chronic lower abdominal and pelvic pain

• pain before or during menstruation

• pain during sexual intercourse

• pain during bowel movements or urination, especially during menstruation

• heavy or irregular bleeding

• back and lower back pain

• bloating and other cycle-dependent bowel complaints

• unfulfilled desire to have children

Endometriosis and fertility

Endometriosis can impair fertility. Inflammation, adhesions, or changes in the ovaries and fallopian tubes can make pregnancy difficult. However, many women with endometriosis can become pregnant even without or with appropriate medical support.

Diagnosis and treatment

Clarification includes a thorough gynecological examination and often an ultrasound. Depending on the symptoms and location, a magnetic resonance imaging (MRI) may be useful. In certain cases, a laparoscopy (abdominal endoscopy) is performed.

Treatment depends on symptoms, extent of the disease, age, and desire to have children. Among others, the following are used:

• pain therapy

• hormonal treatment

• surgical removal of endometriotic lesions

• treatment of adhesions

• in case of desire for children, reproductive medicine measures if necessary

Endometriosis can recur after treatment and often requires long-term gynecological care.
