Ulcerative Colitis (UC) – Chronic Inflammatory Bowel Disease

Ulcerative colitis (UC) is a chronic inflammatory disease of the large intestine (colon). The inflammation typically begins in the rectum and can spread continuously over varying lengths of the colon.

In contrast to Crohn's disease, the inflammation in ulcerative colitis is primarily confined to the mucous membrane of the large intestine and generally spreads contiguously.

What symptoms can occur?

Typical symptoms include:

• recurrent diarrhea

• bloody, mucous stools

• frequent and urgent bowel movements

• abdominal pain and cramps

• painful urge to defecate (tenesmus)

• nocturnal bowel movements

• fatigue and exhaustion

• weight loss in severe cases

• fever during severe flare-ups

The disease often progresses in flare-ups with active inflammatory phases and periods with fewer or no symptoms.

Which areas of the intestine can be affected?

Depending on the extent, distinctions include:

• Proctitis: only the rectum is affected

• left-sided colitis: inflammation extending into the left large intestine

• extensive colitis or pancolitis: large parts or the entire large intestine are affected

Symptoms outside the intestine

Ulcerative colitis can also be associated with inflammation outside the digestive tract, for example, in the:

• joints

• skin

• eyes

• liver and bile ducts

An important co-morbid condition is primary sclerosing cholangitis (PSC), a chronic inflammatory disease of the bile ducts.

Diagnosis

To confirm the diagnosis, the following may be performed:

• blood tests

• inflammation markers

• stool tests, especially Calprotectin

• exclusion of infectious causes

• colonoscopy

• tissue samples (biopsies)

Colonoscopy with histological examination plays a central role in confirming the diagnosis.

Treatment

The goal of treatment is to control acute inflammation, achieve the longest possible remission, and prevent complications.

Depending on the extent and severity, treatments may include:

• 5-aminosalicylates such as mesalamine

• corticosteroids for the treatment of certain acute flare-ups

• immunosuppressants

• biologics

• targeted modern immunotherapies

In severe cases that cannot be adequately treated or in certain complications, surgical removal of the colon may be necessary.

Severe acute flare-up

Severe ulcerative colitis can be accompanied by frequent bloody diarrhea, fever, anemia, circulatory problems, and a strong feeling of illness, potentially requiring inpatient treatment.

A rare but dangerous complication is toxic megacolon, in which the large intestine expands significantly, and a severe systemic inflammatory reaction can occur.

Ulcerative colitis and colon cancer risk

In long-standing and extensive ulcerative colitis, the risk of colon cancer may be increased. Therefore, depending on the duration of the disease, its extent, and individual risk factors, regular surveillance colonoscopies are necessary.
