Cholelithiasis – Gallstones / Cholesterol Stones

Cholelithiasis refers to the presence of gallstones in the gallbladder or bile ducts. The most common form are cholesterol stones, which form when the composition of bile changes and cholesterol crystallizes.

Gallstones can remain asymptomatic for many years. However, if a stone enters the gallbladder or common bile duct and obstructs bile flow, biliary colic, inflammation, and other complications can arise.

Cholesterolemia and Gallstones

Hypercholesterolemia, i.e., elevated cholesterol levels in the blood, is not the same as an elevated cholesterol content in the bile.

Cholesterol stones form due to a combination of various factors, particularly supersaturation of bile with cholesterol, crystal formation, and impaired gallbladder emptying. High blood cholesterol alone does not therefore explain the formation of gallstones.

What types of gallstones are there?

They are mainly distinguished as:

• Cholesterol stones – most common form

• Pigment stones

• Mixed gallstones

The stones can be located in the gallbladder or pass into the bile ducts.

What symptoms can occur?

Many gallstones cause no symptoms.

For symptomatic gallstones, the following may occur:

• Severe pain in the right upper abdomen

• Colic-like pain

• Pain radiating to the back or right shoulder

• Nausea and vomiting

• Pressure or feeling of fullness

• Symptoms particularly after meals

A typical biliary colic often occurs when a stone temporarily blocks the outflow from the gallbladder.

Stone in the Common Bile Duct – Choledocholithiasis

If a gallstone enters the common bile duct (ductus choledochus), bile flow can be obstructed.

Possible signs include:

• Jaundice (icterus)

• Dark urine

• Pale or discolored stool

• Upper abdominal pain

• Elevated liver and bile values

An obstructed bile duct can lead to acute cholangitis.

Possible Complications

Gallstones can cause, among other things:

• Biliary colic

• Cholecystitis – inflammation of the gallbladder

• Cholangitis – inflammation or infection of the bile ducts

• Biliary pancreatitis – inflammation of the pancreas due to a gallstone

• Obstruction of the common bile duct

Diagnosis

The most important examination when gallstones are suspected is usually an abdominal ultrasound.

Depending on the findings, additional tests may be necessary:

• Blood tests

• Liver and bile values

• MRI/MRCP

• Endosonography

• If stones are in the common bile duct, possibly ERCP

Treatment

Asymptomatic gallstones often do not require treatment.

For recurrent typical symptoms or certain complications, surgical removal of the gallbladder (cholecystectomy) is the standard treatment.

Stones in the common bile duct can often be removed endoscopically using ERCP.

Medicinal dissolution of cholesterol stones with ursodeoxycholic acid is only suitable for selected patients and certain types of stones and is not suitable for all gallstones.

In homeopathy according to Dr. Banerji, specific homeopathic remedies and potencies are used for various diseases and complaints of the bile ducts according to the Banerji Protocols.

A reliable dissolution of gallstones, elimination of an obstructed bile duct, or prevention of serious complications through homeopathic remedies is not scientifically proven. Therefore, necessary gastroenterological or surgical treatment should not be replaced or delayed.

Important Note

Severe or persistent upper abdominal pain, fever or chills, jaundice, dark urine, or pale stool can indicate a bile duct obstruction or inflammation and must be medically clarified quickly.

The combination of upper abdominal pain, fever, and jaundice can indicate acute cholangitis and is particularly urgent.
