Cholangitis – Inflammation of the Bile Ducts in Case of Biliary Obstruction
Cholangitis is an inflammation, or more commonly a bacterial infection, of the bile ducts. Acute cholangitis often arises when the flow of bile is hindered or completely blocked, leading to a build-up of bile in the bile ducts.
Acute bacterial cholangitis can rapidly develop into a severe, life-threatening infection, even sepsis, and therefore requires timely medical treatment.
How does cholangitis develop?
The most common cause of acute cholangitis is a gallstone in the common bile duct (choledocholithiasis).
Other possible causes of biliary obstruction include:

gallstones
-
narrowing (stenoses) of the bile ducts
-
scars after operations or inflammations
-
tumors of the bile ducts
-
pancreatic tumors
-
changes in the area of the papilla
-
problems with bile duct stents
-
other bile duct diseases
Typical Symptoms
The classic combination is known as Charcot's triad:

pain in the upper right abdomen
-
fever or chills
-
jaundice (icterus)
Additionally, the following may occur:

nausea and vomiting
-
dark urine
-
pale or discolored stools
-
itching
-
pronounced feeling of illness
-
weakness
Not all three classic symptoms appear simultaneously in every patient.
Severe Cholangitis – Risk of Sepsis
In severe cholangitis, the following may additionally occur:

drop in blood pressure
-
rapid pulse
-
confusion or impaired consciousness
-
circulatory problems
-
increasing weakness
These signs can indicate sepsis and represent a medical emergency.
Diagnosis
The following may be performed for clarification, among others:
-
blood count and inflammatory markers
-
bilirubin and liver values
-
blood cultures
-
ultrasound of the upper abdomen
-
CT
-
MRI/MRCP
-
ERCP (endoscopic visualization and treatment of the bile ducts)
It is crucial to determine the cause and location of the biliary obstruction.
Treatment
In acute bacterial cholangitis, antibiotics are usually required.
If there is a concomitant biliary obstruction, the bile flow often needs to be restored. This is often done by ERCP. For example:

gallstones can be removed
-
narrowings can be widened
-
stents can be inserted
-
blocked bile ducts can be drained

In severe cholangitis, urgent relief of the bile ducts may be necessary.
Cholangitis is not the same as cholecystitis
Cholangitis affects the bile ducts.
Cholecystitis, on the other hand, is an inflammation of the gallbladder. Both diseases can be caused by gallstones but are different clinical pictures.
Homeopathy according to Dr. Banerji
In homeopathy according to Dr. Banerji, specific homeopathic remedies and potencies are used for different disease and symptom patterns according to the Banerji protocols.
Acute bacterial cholangitis or a blocked bile duct must not be treated exclusively homeopathically. Reliable elimination of a mechanical biliary obstruction or a bacterial infection by homeopathic remedies is not scientifically proven. Necessary antibiotic and endoscopic or surgical treatment must not be delayed.
Important note
The combination of fever or chills, jaundice, and severe upper abdominal pain can indicate acute cholangitis and must be immediately medically evaluated.
