Herniated Disc – Disc Protrusion / Disc Prolapse

A herniated disc, also called a disc protrusion or disc prolapse, occurs when disc tissue protrudes beyond its normal boundary, which can irritate or compress nerve roots or other structures of the spinal canal.

Herniated discs are particularly common in the lumbar spine (LWS) and less common in the cervical spine (HWS).

What happens during a herniated disc?

The intervertebral discs lie between the vertebrae and act as elastic buffers. They consist of an outer fibrous ring and a softer, jelly-like nucleus.

Due to wear and tear, aging processes, or stress, the fibrous ring can be weakened. Disc tissue can bulge or protrude.

Distinctions include:

• Disc protrusion: Bulging of the intervertebral disc

• Disc prolapse / herniated disc: Protrusion of disc tissue

• Sequester: Detached disc material in the spinal canal

Not every herniated disc causes symptoms.

What symptoms can occur?

Depending on the location and affected nerve, possible symptoms include:

• Back or neck pain

• Pain radiating into the arm or leg

• Sciatica with involvement of lumbar spine nerves

• Tingling or pins and needles

• Numbness

• Muscle weakness

• Restricted mobility

• Rarely, paralysis

In the case of a cervical herniated disc, symptoms can radiate into the shoulder, arm, or hand, for example.

Herniated disc in the lumbar spine

A herniated disc in the lumbar spine can irritate a nerve root of the sciatic nerve. Typical symptoms include pain that can radiate from the lower back or buttocks down the leg to the foot.

Additionally, sensory disturbances or loss of strength may occur.

Diagnosis

Diagnosis is based on symptoms and a neurological or orthopedic examination.

In case of corresponding symptoms or neurological deficits, a magnetic resonance imaging (MRI) may be necessary. This can show which disc is affected and whether nerve structures are being compressed.

Treatment

Many herniated discs can be treated without surgery. Therapy depends on pain, neurological symptoms, and the extent of nerve compression.

Possible measures include:

• Adapted movement and as short rest periods as possible

• Pain-relieving and anti-inflammatory medications

• Physiotherapy

• Targeted muscle strengthening

• If necessary, infiltration treatments

Surgery may be necessary, especially in cases of severe or progressive neurological deficits, certain persistent severe symptoms, or significant nerve compression.

Cauda Equina Syndrome – Emergency

In rare cases, a large herniated disc in the lumbar spine can compress several nerves in the spinal canal simultaneously.

Warning signs are:

• New problems with urination or urinary retention

• Loss of bladder or bowel control

• Numbness in the buttocks, genitals, or inner thighs ("saddle anesthesia")

• Sudden onset or worsening paralysis of the legs

This can indicate Cauda Equina Syndrome and requires immediate medical or neurosurgical clarification.
