Ascites – Fluid accumulation in the abdominal cavity / abdominal dropsy

Ascites refers to an abnormal accumulation of free fluid in the abdominal cavity. Colloquially, it is also referred to as abdominal dropsy.

Ascites is not an independent disease but a symptom of an underlying condition. The cause should therefore always be medically investigated.

What causes ascites?

The most common cause is advanced liver disease with liver cirrhosis and portal hypertension. This increases pressure in the portal venous system, and fluid can accumulate in the abdominal cavity.

Other possible causes include:

• Liver cirrhosis

• Portal hypertension

• Heart failure

• Severe kidney diseases

• Tumor diseases or peritoneal metastases

• Pancreatic diseases

• Certain infections

• Severe protein deficiency

• Rarely other vascular or metabolic diseases

What symptoms can occur?

Depending on the amount of fluid, the following may occur:

• Increasing abdominal circumference

• Tense or bulging abdomen

• Pressure and feeling of fullness

• Rapid weight gain due to fluid

• Loss of appetite

• Early satiety

• Shortness of breath due to pressure on the diaphragm

• Restricted mobility

• Additionally, water retention in the legs

Diagnosis – It is essential to clarify the cause

Investigations may include:

• Physical examination

• Ultrasound of the abdomen

• Blood tests

• Assessment of liver and kidney function

• CT if necessary

• Ascitic tap with fluid analysis

Especially for newly occurring ascites, diagnostic tapping is often important to clarify the cause and potential complications.

Treatment

Treatment depends on the underlying disease and the amount of fluid.

Depending on the cause, the following may be used:

• Adjusted salt intake

• Diuretics (dehydrating medications)

• Treatment of the underlying liver, heart, or kidney disease

• Therapeutic ascitic tap (paracentesis) for larger fluid volumes

• For certain patients, further procedures to treat portal hypertension

For tumor-related ascites, treatment depends on the underlying tumor disease and symptoms.

Note on ascites infection

In patients with liver cirrhosis, ascitic fluid can become infected. A so-called spontaneous bacterial peritonitis is a serious complication.

Warning signs may include:

• Fever

• New or increasing abdominal pain

• Deterioration of general condition

• Confusion

• Circulatory problems

This requires rapid medical investigation and treatment.
