Heart Attack – Acute Coronary Event / Acute Coronary Syndrome

A heart attack (myocardial infarction) is an acute heart condition in which a part of the heart muscle suddenly no longer receives sufficient blood and oxygen. The cause is usually a complete or partial blockage of a coronary artery.

A heart attack is part of the acute coronary syndrome (ACS) and is a medical emergency.

How does a heart attack occur?

The most common cause is coronary heart disease (CHD), in which atherosclerosis causes deposits (plaques) to form in the coronary arteries.

If such a plaque ruptures or is damaged, a blood clot (thrombus) can form at this site. This can partially or completely block the vessel. The heart muscle behind it no longer receives enough oxygen, and heart muscle cells can die.

What does acute coronary syndrome mean?

The term acute coronary syndrome encompasses various sudden circulatory disorders of the heart muscle. These include, in particular:

• unstable angina pectoris

• NSTEMI (heart attack without typical ST elevation in the ECG)

• STEMI (heart attack with ST elevation in the ECG)

What symptoms can occur?

Typical symptoms are:

• severe pain, pressure, or tightness in the chest

• pain radiating to the arm, shoulder, back, neck, or jaw

• shortness of breath

• cold sweat

• nausea and vomiting

• dizziness or weakness

• feelings of anxiety or restlessness

Not every heart attack causes classic symptoms. Particularly in older people, women, or people with diabetes, the symptoms may be less typical.

Diagnosis and Treatment

Diagnosis typically involves an electrocardiogram (ECG) and blood tests for specific cardiac markers such as troponin.

The goal of treatment is to restore blood supply to the heart muscle as quickly as possible. Depending on the findings, this is primarily achieved through a cardiac catheterization with widening of the blocked vessel and, if necessary, insertion of a stent, as well as through appropriate medications.

A heart attack is a medical emergency

If a heart attack is suspected, emergency services must be contacted immediately. The sooner blood flow to the heart muscle is restored, the more heart muscle tissue can be saved.
