Pruritus sine materia – Itching without visible skin changes
Pruritus sine materia refers to itching for which no visible skin condition is initially identifiable as the cause. The skin appears largely normal at first, even though there is sometimes severe itching.
Repeated scratching can subsequently lead to secondary skin changes such as scratch marks, redness, scabs, thickening, or small wounds. These changes are then a consequence of the itching and not necessarily its original cause.
Typical symptoms
The itching can:
-
be localized or occur over the entire body
-
be constant or intermittent
-
worsen in the evening or at night
-
be intensified by warmth
-
significantly impair sleep
-
be accompanied by burning, tingling, or stinging sensations
In cases of long-standing pruritus, a pronounced itch-scratch cycle can develop.
Dry skin as a common cause
Even if no distinct skin disease is visible, dry skin (xerosis cutis) can cause itching.
This is particularly common:
-
in older age
-
in winter
-
with dry indoor air
-
after frequent or hot showers
-
when using highly degreasing cleansing products
Possible internal causes
In cases of persistent itching without an identifiable skin condition, systemic or internal medical causes should also be considered.
These include, among others:
-
liver and bile duct diseases, especially cholestasis
-
chronic kidney diseases
-
thyroid disorders
-
iron deficiency
-
diabetes mellitus
-
certain blood and bone marrow diseases
-
certain tumor diseases
-
neurological diseases
In some diseases, itching can occur even before other symptoms become noticeable.
Medications as a possible cause
Medicines can also trigger itching, sometimes without a typical rash.
Therefore, in cases of new onset pruritus, it should be checked whether there is a temporal connection with a newly started or changed medication. However, medications should not be discontinued on one's own.
Neuropathic itching
Itching can also arise from changes or damage to the nervous system.
Indications of neuropathic pruritus can be additional sensations such as:
-
tingling
-
burning
-
stinging
-
numbness
-
electrifying sensations
Such symptoms can occur, for example, in certain nerve damages or spinal diseases.
Psychological and psychosomatic factors
Stress and psychological strain can significantly intensify existing itching. Primary psychogenic itching is also possible but should only be assumed after adequate medical clarification of other causes.
Diagnosis
For long-standing pruritus sine materia, depending on the situation, the following may be advisable:
-
detailed examination of the entire skin
-
review of medications taken
-
blood count
-
iron status or ferritin
-
liver and bile values
-
kidney function values
-
thyroid values
-
blood sugar
-
depending on the findings, further internal medicine or neurological examinations
The diagnostics depend on the age, duration, and severity of the itching, as well as possible accompanying symptoms.
Treatment
Treatment is preferably based on the underlying cause.
Supportive measures for dry or sensitive skin may include:
-
regular re-greasing skin care
-
mild, skin-friendly cleansing products
-
short and not too hot showers
-
avoidance of strong heat
-
cooling measures
-
loose, skin-friendly clothing
-
avoidance of scratching
Depending on the cause and intensity, various drug treatments for chronic pruritus are also available.
Homeopathy according to Dr. Banerji
In homeopathy according to Dr. Banerji, specific homeopathic remedies and potencies are used according to the Banerji protocols for various forms of itching.
A reliable treatment of the underlying cause of pruritus sine materia with homeopathic remedies is not scientifically proven.
Since itching without visible skin changes can, in some cases, be a manifestation of a liver, bile duct, kidney, blood, metabolic, or neurological disease, necessary medical clarification should not be replaced or delayed by homeopathic treatment.
Important note
A new onset, severe, or persistent itching for several weeks without identifiable skin changes should be medically investigated.
This applies especially to concomitant weight loss, night sweats, fever, pronounced fatigue, yellowing of the skin or eyes, altered urine, or other general symptoms.
