Atopy refers to a hereditary or genetically predisposed tendency to react to certain environmental substances with an exaggerated allergic immune response. People with an atopic predisposition are more likely to develop conditions such as neurodermatitis (atopic dermatitis), allergic rhinitis, or allergic asthma.

Atopy is not to be equated with a single allergy. Rather, it describes an increased propensity for developing certain allergic diseases.

What is an allergy?

In an allergy, the immune system reacts to normally harmless substances – known as allergens – with an exaggerated immune response.

Common allergens include:

  • Pollen

  • House dust mites

  • Animal allergens

  • Molds

  • Certain foods

  • Insect venom

  • Latex

  • Certain medications

What does atopy mean?

Individuals with an atopic predisposition often have an increased tendency to produce IgE antibodies against certain environmental allergens.

Typical atopic diseases include:

  • Atopic dermatitis (neurodermatitis)

  • Allergic rhinitis (hay fever)

  • Allergic rhinoconjunctivitis

  • Allergic asthma

These conditions can occur individually or together.

Atopic Dermatitis / Neurodermatitis

Atopic dermatitis is a chronic inflammatory skin condition in which both the skin barrier and the immune system play an important role.

Typical symptoms include:

  • Dry skin

  • Intense itching

  • Red and inflamed skin

  • Flaking skin patches

  • Eczema-like changes

  • Weeping skin patches during acute flare-ups

  • Thickened skin with prolonged course

The disease often runs in episodes.

Possible triggers and exacerbating factors

For individuals with an atopic predisposition, symptoms can be influenced by, among other things:

  • Pollen

  • House dust mites

  • Animal allergens

  • Certain foods in the case of a confirmed food allergy

  • Skin irritations

  • Unsuitable care products

  • Dry air

  • Heavy sweating

  • Infections

  • Stress

Not every suspected trigger is actually an allergy. Food, in particular, should not be avoided long-term without a confirmed diagnosis.

Atopic March

In some people, atopic diseases change over the course of life. For example, atopic dermatitis may first appear in childhood, later followed by allergic rhinitis or asthma.

This possible progression is known as the "atopic march". However, it does not occur in every person with atopy.

Diagnosis

Diagnosis depends on the specific symptoms and may include:

  • Detailed medical history

  • Skin examination

  • Prick test

  • Determination of specific IgE antibodies

  • Epicutaneous patch test for suspected contact allergy

  • Lung function test for suspected asthma

  • Controlled provocation tests for specific questions

A positive allergy test alone does not automatically mean that a clinically relevant allergy exists. The connection between the test result and actual symptoms is crucial.

Treatment

Treatment depends on the specific atopic or allergic disease.

Possible measures include:

  • Avoiding known relevant allergens as much as possible

  • Consistent skin care for atopic dermatitis

  • Antihistamines for suitable allergic symptoms

  • Anti-inflammatory creams or ointments for neurodermatitis

  • Inhaled medications for asthma

  • Nasal sprays for allergic rhinitis

  • For certain allergies, allergen-specific immunotherapy (hyposensitization)

For severe atopic dermatitis or severe asthma, targeted immunomodulatory or biological therapies are also available today.
